Brief Answer:
Muscle strain related pain.

Detailed Answer:
Hello dear, welcome Ask a doctor's service.
I read your query and here is my advice.

If you are having irregular bleeding and discharge and can't feel the IUD strings you may need to visit your gynaecologist.
The strings can even be there and you can't feel them but the discharge, bleeding and back pain may be related to any infection.
The shoulder and neck pain is not much related to that but to muscle pain. The neck and backpain may be related to a muscle strain or injury.
I can advise you to take antinflamatory drugs such advil or acetaminophen for pain. You can use warm compresses on neck and shoulder or can even use diklofenac gel to relieve pain.

Hope I have answered the question.
