Question: Hello, I'm 26 years old man and I have a tinder swelling above my left testicle inside the scrotum, I visited a urologist and he told me that it is a spermatocele and it has no dangerous effects. and told me to ignore it. my question is I have an uncomfortable feeling behind the left testicle, you can see the image, is this feeling caused by the spermatocele? should I be worried about it ? could it be caused by musterbation ? there is a blue circle to show the location of the uncomfortable feeling.

Hi.
Thanks for your query and an attachment.
Noted your history and understood the concerns.
Please give additional information for me to assist you better and answer all your questions.
Since how many days/weeks is this problem present?
The area you have marked in the photo is above and behind the scrotum and not above the testicle. Please give details. The best way is to have an actual photo with marker to tell about the position of the tenderness.
Was any Ultrasound with color doppler done?
The area which you have marked in the diagram is the base of penis - the crura.
Do you do hard masturbation pressing on the base too? Do you use lubrication?
Any other information please.

Awaiting for your feedback.

Thank you for your answer doctor, I'll try to give as many details as I can in this reply. I noticed the swelling above the left testicle a long time ago like months ago but I thought it was normal, 5 or 6 weeks ago I started paying attention to it and I thought I should visit a doctor, especially when I had some uncomfortable feeling in same area in image, actually it was painful for 2 or 3 days. I visited a urologist 2 weeks ago, he checked the swelling manually by pressing on it, and he told me it is a spermatocele, he said I can make an ultrasound but he was pretty sure it is nothing more than spermatocele. so I decided not to make the ultrasound. actually I have hard masturbation sometimes, every 2 days for 2 or 3 times. and I don't use lubrication. sometimes I press on base but not too hard. if I upload a photo with marker is it going to be private? if the ultrasound is necessary I can make it in few days.

As discussed

Detailed Answer:
Thanks for your detailed feedback.
Yes, the name and all your information including the photos are totally private so no worry.
Let us have an Ultrasound first and tell the Radiologist doing the Ultrasound where exactly was the pain and the swelling so that he can get the best possible diagnosis.
Then only post the real photo if required.
Spermatocele is never painful.
Hence I think the painful area can the crura of penis due to hard masturbation which settled and the spermatocele can be an incidental finding. Using lubrication may be a good idea, try it.
Do not press upon the base of penis at all.
Another reason can be boil or so since you had pain.

Hold masturbation till reports.
I hope this answer helps you.
Awaiting for ultrasound report.
