Hello, I Was Bending Over My Wife To Kiss Her
I was bending over my wife to kiss her goodnight and she went to slap me on my chest in a fun way, not fighting. She hit me with an open hand slap, not full force, below my right peck in the upper right abdomen area. There was an unusual pain so I looked it up and it looks like she could have hit my liver directly. It is slightly tender in one small spot in the upper right abdomen two inches or so to the left of my rib cage and one inch below my right peck. I also have a dull pain associated with the area. This happened twelve hours ago. I have no other symptoms other than the slight tenderness and dull pain. No signs of blood loss, shock, etc. No sharp pains. My question is should I go to the emergency room for diagnosis or wait a few days to see if pain and tenderness go away.

Hi.
Thanks for your query and an elucidate history, noted and understood the concerns.
Since 12 hours have passed and you have no other symptoms than the ones you have explained, this looks to be local contusion only.
Internal bleeding causes the severe most pain and one is unable to move or take a deep breath.
But since the area is for liver, gall bladder, colon and duodenum in the anterior part sometimes the internal contusion may manifest late and one can be having the least possible symptoms and signs.
It is always better to get at least an Ultrasonography which can show any internal organ problem, collection or so.
I would surely advise going to the ER.

If tomorrow morning, 15hrs from the current time, I feel no pain, would you still suggest an ER visit?
ER is very expensive with my insurance and I only want to go if needed.

I can understand your point of view.
As explained above only a clinical examination and Ultrasound can tell if there is anything internally also.
Few of the problems show up a bit later hence the necessity to be sure that there is no internal trauma.
Unusual pain as you have mentioned indicates that the hit was strong enough.
Even though there is no pain there can be still an issue and such interval is called lucid period.
Hence the concern.
