Question: Hello, I'm taking 30 mg of Latuda twice a day for schizoaffective disorder, 100 mg of Luvox once a day for OCD and anxiety, 5 mg of propranolol twice a day for tachycardia, and 5 mg of trintellix once a day for depression. Today my doctor added 150 mg of trileptal once a day for mood stabilization. It said on the internet that trileptal increases the levels of trintellix by increasing metabolism. Luvox and trintellix also increase toxicity of each other by serotonin levels. So since both luvox and trileptal increase the levels of trintellix, will this result in an overdose of trintellix or serotonin syndrome?

Luvox and trintellix are kind of medications which increase serotonin and the elevated level of serotonin is responsible for their action in anxiety, depression and obsessive compulsive disorder. It is also true that the metabolism of trintellix is affected with trileptal as trileptal causes induction of enzymes involved the metabolism of trintellix as well as luvox.

It can be concluded from the above description that fear of serotonin syndrome is ground reality if many serotonergic medications are part of prescription. There is definite risk for serotonin syndrome in theoretical terms.

However none of the medications is in high range and practically there is very low risk of serotonin syndrome in your case as the medications were added in gradual manner.

In addition I like to add one suggestions.
1. I do not see any ground for use of trintellix when luvox is already part of prescription as both medications act in similar manner. In opinion increasing dose of luvox is better option rather than adding trintellix for treatment of depression.
