Hello, I Have A 1 Cm Sore In My Nose

Question: Hello,
I have a 1 cm sore in my nose I've been treating with 2x/day mupiocin ointment for 1 week. It still has white puss on the edges and a darker(perhaps scab) in center. Not sure that ointment isb helping although bleeding has mostly stopped. Saw ENT 1 wk ago but he is out sich until next Tuesday. Should I be concerned that this isn't healing properly and visit ER with resident ENT tomorrow?(Saturday) I'm concerned about it being a more serious infection. Thanks!

Hii. Is the sore is within the anterior part of nose or it is outside the nose over the skin? Can u please post a picture? From your history it looks like you are having either a vestibulitis or a small folliculitis. It is mostly due to Staphylococcus infection of the hair follicles of the nose. Continue mupirocin ointment. Don't try to remove the scab or pich it. You can take tablet amoxycillin or augmentin to reduce the bacterial infection. Use nasoclear nose drop to keep the nose moist and remove the crusting. Staphylococcus is a coloniser in the hair follicles. Sometimes the hair follicles get infected by Staphylococcus and folliculitis along with abscess can form. So it is always better to treat it completely with ointment and antibiotics as it can recur again and again
If possible you can send the pus coming from the sore and sent to laboratory for culture and sensitivity and see what is the type of organism responsible for infection and on which drug they are susceptible. Clean the clear with hot saline water slowly and apply the ointment. You can consult ent surgeon when he will be in the town. I'd your are having allergic symptoms with sneezing, rhinorrhoea you can take fluticasone nasal spray and bromhexine tablets. Please review if you have any queries.

It’s inside nose and seems to ooze. How often do these sores progress to a serious infection? Should I go to ER and see on call resident or wait to see ENT on Tuesday? Thank you!

Hii. You can go to ER resident but as you have mentioned that symptoms have regressed then you can continue the antibiotic and ointment and meet ENT resident on Tuesday. You can continue nasockear nose drops. On these medications there are less chance to convert this to severe infections.

The sore seems to be the same visually an open lesion about 1 cm wide with some crusting and some white puss. Do you think it would be better to see an experienced ENT Dr in 3 days or see resident who I assume has less expertise for evaluation. I'm using mupirocin 2x/day for 1 week and sore looks maybe a little less pussy. What % of these lesions become life threatening? Thanks very much!
