Question: Hello I am a 35yo male, who had sex with a professional. I did not have insertive sex without a condom, but I am experiencing symptoms which look like a seroconversion. I am unusually very tired and sleepy all the time, I don't have fever but had some chills, I have pain on random joints. I don't have diarrhea. About the lymph nodes is weird, I had a severe jaw ache, which was prediagnosed by a dentist as trigeminal neuralgia or TMJ disorder, I took carbamazepine a few days and that pain is gone now. I don't feel clear swollen, hard nodes on the mandible or neck, but it do feel puffy arround the front of the neck it and hurts were the nodes are supposed to be, sometimes. That pain goes on to the shoulders. Also I feel as if I have something on my armpits when I close them, they are puffy, I have pain along the great dorsal wing that ends of the armpit and have a clear and very localized pain 2 or 3 ribs bellow the right armpit, like a painfull node, the size of a small bean that hurts when I push it. A few days ago I took a certified pharmacy hiv test that appeared negative but later I noticed a dark faint shadow on the test line, which could be a faint positive. I contacted Mylan pharmaceuticals and they said it's almost for sure a negative. Almost... But with these symptoms I am confused and very scared...

You had penetrative sex with the professional but with condom then the risk of getting HIV is not there and the test result for HIV is also negative so don't get scared or confused. I request you to explain the incidence in detail along with the time of test after exposure so that I can guide you in much better way.

As far as your symptoms are concerned, they look like to be of flu like illness due to seasonal change or any other reason but not related to HIV in any way. Thank you for your answer. My concern, when I look at my symptoms, is that the condom have slipped without me noticing or something like that. I don't have fever, at all, although I fill chills and am constantly feeling cold, nor do I have diarrhea or skin rash. What concerns me most is the pain on the places where lymph nodes are supposed to be, and the feeling of those areas being a bit swollen. I have ocasional pain, not very intense, in the exact places where the nodes are. Under my jaw, in the neck soft tissue, along the leck muscles, on my shoulder blades, arround my armpit, and bellow it and arround my ears. I don't palpate any hard lumps, are inflamed nodes supposed to be firm? And on my armpits I feel like there is an object that keeps me from closing my arms completely, specially on the right side. As for timing, the test was made before the time window as passed, I made the test 2 to 3 weeks after the encounter and the usual window for testing is 6 weeks. And I have been monitoring the symptoms ever since. I also have a mildly sore throat, runny nose and persistent dry cough. I attach a photo of the test, one hour after it was made.

The symptoms are typically of common viral or flu like illness due to seasonal change nothing else. The swollen lymph nodes under the jaw and on the sides of neck are most common which can occur due to viral illness or local infection around the area. The attachment of test result is not there so I request to send it again for further advice.

As such no further testing is required but for your mental satisfaction I suggest you to go for repeat test after 12 weeks of exposure to get 100% reliable and conclusive result because the window period for HIV infection varies from 15 days to 3 months. Calm down, allay the fear or anxiety, don't think unnecessarily and live your life normally.

Thank you for answering. It's not just the neck, but also the armpits, arround the armpits. I'm trying to get that spirit of moving on with my life, but there is some evidence that won't let me. I attached the result again. Are enlarged nodes supposed to be a hard bump or just soft augmented masses?

I don't think that there is any scientific evidence regarding transmission of HIV in the exposure you had mentioned because enlarged lymph nodes are not the bench mark to suspect HIV. Again I advice you to consult the physician to rule out cause for enlarged lymph nodes. The attachment is not there to comment any thing but I don't think you are at risk of HIV any how.
