Question: Hello, I am 49-year-old 5.4 inch 54KG. I have been Type 2 diabetic for last 16 years, most recent A1c is 7.0. I take 2000 MG XR Metformin per day (1000MG Twice a day). I am active person. My questions is why do I feel hungry even though when my blood sugar showing up 192, two hours post meal. I have been taking metformin for last 16 years, does its effect reducing ? should I think about adding some other class of drug? I really wanted to avoid adding any other medication instead I rather do more workout. I try to do 30 to 45 min 3 times a week work-out.

I have gone through your query and understood your concern.

Your recent HbA1C level show that you have a well-controlled diabetes.
This also means that your average blood sugars are between 140 to 154 mg/dL.

With these results your treatement with Metformine 2000 mg/day can be considerated Ok!You do not need to add other diabetes drug ,especial if you try to do at leat 30 min workout, 3 times /week.

About the hungry sensation i can say that in uncontrolled diabetes where blood glucose levels remain abnormally high , glucose from the blood cannot enter the cells – due to either a lack of insulin or insulin resistance – so the body can't convert the food you eat into energy.
This lack of energy causes an increase in hunger.

In your case ,hungry can not be explaned with abnormaly high blood sugar levels because you have a well controlled diabetes.
YOu should know Eating more will not help.

The solution is to "Increase" insulin sensitivity (the way your cells respond to insulin) with medications (like your metformine)and/or lifestyle changes such as healthy weight loss and more physical activity.

Thank you for your reply, in my case weight loss is not an option as i am already lean with only 54KG Weight.

I heard one way to increase Insulin sensitivity is work-out, are there any other ways to increase insulin sensitivity? (Certain foods or vitamins etc.?)

Yes,fortunately, there are many things you can do to naturally increase your insulin sensitivity.
Apart regular exercise and cutting down on carbs, a good night sleep, eating more soluble fiber,reduce stress,drinking a lot of water and a couple of green tea cups every day will be helpful to improve your insulin sensitivity.

Foods that are rich in soluble fiber include legumes, oatmeal, flaxseeds, vegetables like Brussels sprouts and fruits like oranges.

In particular, colorful fruits and vegetables are rich in plant compounds that have antioxidant properties(the antioxidants help increase insulin sensitivity.)
But be careful not to eat too much fruit in a single sitting, as some types are high in sugar.
Also,turmenic,ginger,cinnamon,apple cider vinegar are important food that can be helpful for your insulin sensitivity.

About the supplement,the idea of taking natural supplements to increase your insulin sensitivity is fairly new.
