Question: Hello
I am 30 year old male, quit smoking 3 years ago, occasional drinker, last time I drank was 8 months back. For last 3 weeks I have been having rectal inflammation and pinworm infection like itching, which gets worse when I seat. Last week I found fresh blood with stool. I went to the doctor who gave me Venusmin 150 and a XXXXXXX called Relax to soften the stool. Since then there's no blood in stool. But the rectal inflammation and itching persists. I've done stool and blood tests. Results are below;

Stool test
appearance - soft brown stool
visible blood - nil
mucus - present
reaction - acid
ova - nil
larva- nil
entamoeba histolytica veg - nil
entamoeba coli cyst - a few
giardia lamblia cyst - nil
trichomonas hominis - nil
iodamoeba butshilli - nil
pus cells - fair number
epithelial cells - nil
rbc - nil
macrophage - nil
fat droplets - nil
undigested starch - +
vegetable cells - ++

Blood test
haemoglobin - 12.8
pcv - 38
wbc - 7800
neutrophil - 62
lymphocyte - 32
monocyte - 1
eosinophil - 5
basophil - 0
ESR - 1st hour 20

fyc, I did a blood test 4 years ago and then my haemoglobin was 12.9 and pcv was 38. I have occasional inflammation of the scrotum and testicles and ilium, it is mild and normally subsides. I don't have abdominal problems like cramps or pain. I sometimes have morning nausea but it subsides once I take food. I never had unprotected sex, so you can rule out STDs.

Brief Answer:
Get a Sigmoidoscopy.

Detailed Answer:
Hi,

Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

Your stool test and complete blood count are not showing anything significantly abnormal.
Bleeding per rectum can occur in number of conditions like hemorrhoids, anal fissure, infectious colitis, inflammatory bowel disease and malignancy.

I would advise you to get a sigmoidoscopy test done to find the cause.

The other symptom is pruritus ani. Some of the causes of pruritus ani are hemorrhoids, fissure, fecal incontinence, candidiasis, parasites, diabetes, coffee, milk, beer. Drinking coffee is considered the most common culprit. So you need to stop drinking coffee. Examination should be done to look for the conditions mentioned above. You can discuss with your doctor about these possibilities and the tests.

I hope that answers your question.
Let me know if I can assist you further.
Regards,
Dr.R.K.

Hello

I don't have diabetes because my sugar level is normal, i.e. 74 and I don't drink coffee. Thanks for your suggestion of getting a sigmoidoscopy. I'd like to know if I need to do colonoscopy also or sigmoidoscopy is sufficient.

Thanks

I'd also like to mention I have one or two bowel movements a day and for last two days, I've found very very small solid red particles in stool. I'm not sure what they are. I'm also having frequent urination for last two days...any thought on what these symptoms point at?

Brief Answer:
Would like to have a look.

Detailed Answer:
Hi,

I would like to have a look at the red particles before I can give my opinion.
Frequent urination most often indicates urinary tract infection and diabetes mellitus.
Urinalysis and blood glucose test may be done to find if any of these is causing frequent urination.
But you should also know that increased intake of fluids and a low environmental temperature can also cause frequent urination.

Let me know if I can assist you further.

Regards,
Dr.R.K.
