Question: Hello Everyone,
I'll try to be brief but concise. I'm 57 years old and have already gone through menopause. For the past couple of years, I've been experiencing extremely light bleeding. Now, the bleeding is not constant. It is very sporadic. Meaning that it's only happened about four times. And each time, it doesn't last days or anything like that. I see it once, then it's gone. And again, it's extremely light. However, because I suffer from OCD and health anxiety, I always get myself checked. Since this started happening, I've been to my GYN for my regular check ups. She's done four biopsies (each time I have seen blood), ultrasounds and one D&C. The last ultrasound was this past December (2019) and my last biopsy was this past March (5 months ago). All the tests that she's performed have been normal. I do have fibroids and a polyp. My anxiety is kicking in and I am feeling depressed because this past Friday I had minor bleeding again. It was extremely light and again, it didn't last. I saw it once and as I continued with my day I never say any more bleeding.

I did call my doctor on Friday but she wasn't in. I'm calling her tomorrow. I'm so confused and full of anxiety. What can it be? I'm just afraid it's cancer. I know that we are going to die of something, but my fear is that I just don't want to die too young (well, I'm sure no one does, of course).

Can fibroids make you bleed? Can a polyp make you bleed?

I know you can't diagnose but I need some guidance. What should the doctor perform next?

thanks for the query to Ask A Doctor service,
In your case, as you have already got your biopsy and ultrasound pelvis done for investigating post menopausal bleeding and it has been normal, then unlikely that you would have either cervical or endometrial cancer else either a pap smear or a endometrial biopsy for histopathology would have picked it up.
The only other cause for the bleeding could be an intrauterine polyp or atrophic vaginitis per se itself.
The next best step would be to repeat a pap smear and check for inflammatory smear or atrophic vaginitis.

thanks for the query to Ask A Doctor service,
In your case, as you have already got your biopsy and ultrasound pelvis done for investigating post menopausal bleeding and it has been normal, then unlikely that you would have either cervical or endometrial cancer else either a pap smear or a endometrial biopsy for histopathology would have picked it up.
The only other cause for the bleeding could be an intrauterine polyp or atrophic vaginitis per se itself.
The next best step would be to repeat a pap smear and check for inflammatory smear or atrophic vaginitis.

She'll do the pap smear then because I'm due for it next month anyway. However I'm still quite nervous. Do you think it's wise to wait or should I see if I can find another doctor. Thanks for a follow up,
I really don't think that its would be a delay if you have to wait for your doctor till september 25th for a repeat pap smear as i am suspecting cervicitis and/or atrophic vaginitis alone and not any malignancy, so it is anyways better to follow up with a doctor who knows your history and has been primarily looking after you.
In the meantime , you can opt for alkaline vaginal washes to keep the local area clean and also opt for oestrigenic foods like soyabean , alpha sprouts , tofu, flax seeds... which would help your natural estrogens to build up and help improve post menopausal symptoms and atrophic vaginitis as well( by improving the vaginal mucousa).

Hello again doctor. Hope all is well. Just wanted to let you know that my doctor scheduled me for an ultrasound on September 9th. The last ultrasound I had was December 2019 and that came out normal period my uterine lining was normal. I'm still a little scared but based on everything that I've told you even if this time around the uterine lining comes back a little thicker I hope it's not something that I should be extremely worried about. Hopefully if it is I hope it's nothing but if it is something hopefully it's something that can be taken care of period what are your thoughts on this doctor? my lining was normal back in December and my biopsy was normal this past march. What are your thoughts?

Apologies for late reply. I think any opinion can be formed only after an ultrasound pelvis and a pap smear examination. I guess what I'm trying to tell myself in order to not be so scared is that I'm sure there's lots of women going through something like this. I also wanted to ask that God willing it is not cancer what other medical issues could be happening so that I bleed very lightly? One of the questions I have is that how common is this in women? I know you said probably the parlor for the vaginal atrophy . But would it only be those two? Would there be any other issues where a woman would have light bleeding and not have cancer? Thank you again doctor. I am so worried. I am actually crying right now. I've done everything right period as soon as I've had the light bleeding I've been to the doctor each and every time and I've had a biopsy done each and every time period and thank God everything has come out normal.
