What Causes Breathing Difficulty?

Question: Hello Dr.,
I'm 43, on blood pressure medication, borderline diabetic, bad history of drinking, because i suffer from premature ejaculation, so i drank alot to be prepared sexually, I still exercise 3 times a week, jog-pushups etc, and i do mix martial arts. i do have a beer gut. Why is it that when people get on top of me, i can't breathe, in mma when i get on top of people, they can breathe, it's like when you get on top of me in a certain place of my chest i can't breathe, and now in certain excercises i do, i can't breathe, i feel alot of pressure in my ears too, sometimes, after sparring, i have shortness of hearing, and yes it goes away after a while, i hear so many rumors, collapse lung, liquid in lung, i feel if there is something swollen in my stomach that prevents my lungs from expanding, i tried inhalers, they help little

Thanks for the query.
I have read your query  and understand your concerns very well.
Your symptoms suggest that either your symptoms are either because of cardiac involvement or may be because of excess weight ( if you are overweight )
Blood tests of heart don't reveal the status of heart.
You need to consult a cardiologist and get a echocardiogram and TMT test done to know the exact situation.
Inhalers won't help if your lungs are normal ( MRI lungs is normal )
Excess of alcohol intake is bad as it damages heart,it doesn't let you control your sugar levels,it causes gastritis and heart burns and may give a feeling of swelling in stomach  and also it won't let you loose weight.
You must cut down your alcohol.

Ok, but where does the pressure in my ears come from? The partial deafness after sparring? And can I delete liquid in my lungs or collapse lung?

Your symptoms could be because of high blood pressure and some deranged cardiac functions.As far as lungs are concerned,they are normal since MRI lungs doesn't reveal 
any abnormality.
