Question: Hello Dr,

I am Rajesh G and i am raising this query for my Father in Law.

Name : Natarajan V
Age: 59

My father in law has been having severe constipation for the past 30+ years. He used to excrete once in two days for the last several years.

However the condition got worsened and couldn't excrete for more than 5 days and couldn't; t pass urnir for more than 1.5 days. He was unable to consume food and became very week. We took him to the nearest hospital for consultation and taken the following tests. Based on the test results, we were informed that my Father in law has the following issues.

1) Fatty Lever,
2) Sediments in the Urinary Bladder,
3) Earnea and needs to undergo surgery.

Queries:
1) Do we need to take any medicines for Fatty lever
2) Does the Sediments on the Urinary bladder needs any minor surgery.
3) Doe the earnea requires any surgery.

Requesting you to please refer the attached reports and please provide your expert advise. Based on the recommendation, we would consult specialist for the next line for treatment.

Welcome to Ask A Doctor services.
I have read your query with attached reports of laboratory and ultrasound and here is my advise.

* The current issue of abdomen pain with dysuria are related to severe
constipation with secondary effects.

The size of prostate is not that much significant to stop urination , fatty liver is mild and urinary sediments in bladder are secondary to urinary tract infection.

My guidelines at present

- Umbilical hernia of 2.5 cm is not the indication of an emergency surgery
unless there is severe obstruction of intestines with uncontrollable
vomiting.
- He requires control of blood sugar which is not under control and that will
lead to sluggish movements of his bowels via neurologic mechanics.
- He has to consult his diabetic specialist for vigilant control of sugar which
will help him over 50 - 70 percentage control of constipation and urinary
infection as well.
- Get prescription of antibiotics for urine infection as per the urine culture
report.
- Consider use of soap water enema for immediate evacuation of bowels and
bulk forming laxative as Ispaghula husk or else for chronic constipation.
- He must remain ambulatory to help bowel function.

Welcome for follow up.

* Soap water enema is medicated liquid filled in sterile plastic bag and is
administered through anus opening by registered nurse or doctor, through
delivery of this liquid, the hard stool in lower colon and rectum will
become soft and would be easy for him to defecate.

Later on Isabgul or other stuffs will work better way.

* He must remain ambulatory means that he has to keep moving as walking
or daily home activities, if he remains for a long period of time without
activity in bed, then intestines are not going to function better way.
