Question: Hello Doctor,
I recently had some swelling in my groin and i went to see a hernia specialist because i thought that was the cause upom examining me he said that i do have a hernia but he couldn't feel my right testicle aswell so he suggested me to have a mri done and i have done the mri so i wanted to an urologist to have a look at it and give me their opinion

You can upload the MRI report with the help of the customer care team. or write a conclusion part of the report as a text in follow up.

Assuming that it is undescended testis you need to consult a General surgeon or Urologist for clinical assessment and to confirm the diagnosis.

Ideally, undescended testis has to be repositioned and fixed up in scrotum before the age of 3-4 years. Since you are 24 years old it is highly unlikely that it is functioning now and also carries a high risk of developing some other pathologies, hence it is always advisable to remove it.

and I do have a couple of questions about this. first of all the undescended testicle does has to be there from young age or could it have gone up in groin recently. Second how do you perform a test to see if the testicle is still functioning or not. If it is functioning can i be bought down with anything else except surgery.

Undescended Testis is a congenital anomaly and is present from birth.I would explain pathophysiology of the same . In intrauterine life Testicles are intra abdominal organ and they start descending down to be placed in scrotum and descent being completed by the third trimester .This is a very complex process governed by hormonal and anatomical factors .But in one in one thousand cases this process of descent is obstructed by some factors resulting in undescended testis and Testis is retained either in abdomen , near the internal ring or inguinal canal resulting in to what is called as Undescended Testis .In most of cases it gets placed in Scrotum by the age of 6 months or may be placed in scrotum with ease by surgery of (Orchiopexy) up to age of 3-4 years.If neglected the testis at the retained site does not develop and is smaller in size compared to its counterpart placed in normal position (Scrotum).

There is no test to confirm whether the undescended testis is functioning or not .However all the undescended testis after removal by surgery have been proved to be non functional on biopsy examination .

The undescended testis at this age of 2 years can not be brought down as other structures like spermatic cord ,Vas Difference, testicular veins and nerves could not be lengthened so as to reach up to scrotum .

I would suggest you to consult General Surgeon for clinical assessment along with the MRI report .He would be the best judge to decide further treatment .

I am sorry I can not make precise comments and suggestions without MRI report .

Thanks for uploading MRI report which confirms that your Right Testicle is retained in abdomen .You have not mentioned in your earlier discussion about you having been operated for Rt inguinal hernia in childhood .I wonder how the surgeon missed the fact about undescended testis during surgery .

As explained in my earlier reply it is very difficult to mobilize the Testis all the way from pelvis and bring it down to fix up in scrotum at the age of 24 years .However you being in advanced country like U K and in London and your Rt testis being of normal size and devoid of any neoplasm you can explore possibility of preserving the Rt testicle by doing Laparoscopic Evaluation and whether it is technically possible to fix it up in Inguinal canal .However the decision regarding this will be taken by operating surgeon intra operatively during surgery . You may consult renowned Laparoscopic surgeon in London .

If you were to be my patient ( In India)I would have advised you to get it removed considering the fact that your left testis is normal and risk of Rt testis developing other pathology in future being high ..
