Question: Hello Doctor,
I am 38 years old healthy male with no prexisting conditions. I had a urinary tract infection with high fever(103) and chills on December 23. Hospital did urine culture that shows large E.Coli infection and some pseduomonas auergonias infection as well. They gave cipro which seems to have helped a bit. Few days later my primary care physician ordered blood test and also did a renal/bladder ultrasound. The ultrasound was normal and didnt find any stone or prostate enlargement. Two days ago i had some blood in urine again along with burning sensation while passing urine and my primary care physician gave me Cefpodoxime which i am currently taking. My physician did a urine culture today and says no signs of infection but I still have persistent/dull pain in the right lower abdomen/flank. Therefore, they are now suspecting a smaller kidney stone and have asked me to schedule a CT scan. While I wait for the CT scan, can you please help guide me on the following:
1) On the blood test report what do my high Creatinine(1.33 mg/dl), low BUN/Creatinine Ratio (7) indicate?
2) on the blood report, my Bilirubin is normal 0.6 mg/dl, A/G ratio is normal (1.6 with Ablumin -4.6 g/dl & Globulin 2.9g/dl). My AST (SGOT) is normal -34 IU/L. However, my ALT(SGPT) is high -75 and my Alkaline Phosphatase is high (139). Should i be worried about my liver function? or do you suspect it's high because of infection? I have slight belly fat so I do expect some elevation in these numbers.
3) Since yesterday I have been experiencing some dizziness. I have been taking vertin for it. Is this dizzyness worrisome?
4) Do you suspect a kidney stone? or do you suspect appendicitis, hernia, or liver issues?

I am sorry to hear about your troubles. The increased creatinine is probably from infection, alternatively a kidney stone which the CT scan will detect. This will in case of normal BUN lead to a low ratio. The slightly elevated liver tests may be the infection, alternatively caused by medicine or gallstones. I suggest you have an ultrasound.

Hello Doctor,
Many thanks for your kind guidance. Couple of quick follow-up questions:
1) So you don't suspect Hernia or Appendicitis? Are those likely ruled out?
2) I will be undergoing Abdomen/Pelvis CT scan (oral contrast) for Kidney stone detection. Will that detect issues with the Liver/Gallbladder as well? or would you still recommend a separate ultrasound for the liver?

Hi again,

Hernia or appendicitis sounds unlikely. However, these conditions would likely be diagnosed on your CT scan, which I would recommend be carried out also with a phase of intravenous contrast. Sometimes gallstones can be seen on a CT scan, but ultrasound is better.
