Question: Hello Doctor, Good Evening.

This is Anas (37/M) from XXXXXXX and I'm contacting you for my 4 years old son. He is having mild stomach pain since yesterday, but he doesn't know where exactly the pain is. Moreover, there is 1-2 canker sores inside his mouth which started in the last 2-3 days, so we kept coconut oil on canker sores few times daily but no improvement.
We recently changed our tooth paste to Dabur Red (https://www.dabur.com/in/en-us/products/dabur-red-paste) and my son also started using it. This canker sores appeared after changing the toothpaste. We are not sure whether this is relevant point to this problem, but we would like to let you know.
Furthermore, around one week back, he had fever (between 100'F to 102'F) and we gave him Calpol 250 mg syrup 4ml thrice daily and then fever subsided. His temperature did not raise in the last 2-3 days, but he had vomited 1-2 times when he had fever last week. As of now, there is no fever or vomit in the last 2-3 days.
From today (2nd Sep), he started having loose stools problem.
We seek your advice and treatment.

I have gone through your query in detail, and understand your concern.

So the story is- He started having fever & vomiting 1 week back, received Calpol > now afebrile for 2-3 days and no vomiting> but now having mild diffuse abdominal pain for 2 days, and started having loose stools today. Plus, having canker sores for last 3 days.

Going through the story, this seems like an acute gastroenteritis(bowel infection). Most of these are viral, and need only supportive treatment.

You can give him the same dose of Calpol for abdominal pain(mild-moderate pain will respond to this). Give 2-3 times a day as needed. After each loose stools, give him fluids like ORS solution or salted rice water to prevent dehydration. Give in small frequent sips to avoid vomiting. Giving Zinc and a probiotic will hasten recovery. You can give Z&D drops(20mg Zinc/1ml) 1ml once daily for 2 weeks and ECONORM XXXXXXX twice daily for 5 days(these are not a must in mild cases).

Also, avoid fruits, juices, and sugary items till recovery- these can worsen diarrhea. If fever re-appears, stool contains blood or mucus, or he becomes lethargic and not taking feeds, we will need further evaluation and additional treatment including antibiotics. Otherwise, we can wait for 4-5 days for symptoms to settle.

Canker sores will take 7-10 days to resolve. Avoiding spicy food items will help. If significant pain, apply some aphthous ulcer gel 2-3 times daily. Quadrajel is one such preparation.
