Question: I do have excessive belching, burping and releases a lot of gas from my mouth, it is worse in the night several hours after my last meal, the condition makes sleeping difficult. I feel choking sensation when lie down to sleep, sometimes wakes up suddenly gasping for breath. I also sometimes feels dizzy, light headed and palpitations when I experience the episode. I have a feeling sensation of struggling to breath,. I am only able to sleep in the morning when my stomach is empty. I have visited hospital emergency several times and heart problem had been ruled out. I cannot have a good sound sleep. I have taking several anti-biotics regularly for over two years but no improvement in my condition, Daily dose ( Amoxycillin 1g x 2, Tetracycline 500mg x 4 and Omeprazole 20mg x2). for two weeks, then for one month. I also feels mild occasional dull pain below my right breast, very intermittent, sometimes goes away without any feeling. I am on daily 20mg of esomeprazole ( NEXIUM) which I do take in the night before going to bed.What problem do I have?

- Are you overweight?
- What is your nature of work?
- What is your food habit? i.e. type of food, number of meals you take per day with amount and timings.
- Since how many days you are on Amox, omeprazole and tetra?

My weight is about 96kg. I am currently unemployed, just made redundant but I am not in any stress. My main meal is oatmeal in the morning around 11am and rice with fish in the evenings. I always have my dinner around 7pm. I do not take alcohol or drink soda (ie coca cola, spirite, orange juice etc.)

I was given Amoxycillin 1000mg x 2 daily for two weeks 14 days, Omeprazole 20mg X 2 daily for two weeks and Tetracycline 500mg x 4 for two weeks.

Hi,

Yes, the symptoms are most likely due to acid peptic disease. Most probably the medications are given to you for H pylori eradication, if at all it is present in stomach. That organism is responsible for severe acidity and gastritis.

Besides that many a times sleep apnoeas happen in a person who is overweight. If your weight is above the normal recommendation (BMI above 25), this possibility should also be considered. Sleep apnea causes sudden awaking from deep sleep. Sleep deprivation at night again leads to increased acidity and day time sleep.

What I suggests are:
- After completion of your 2 weeks therapy, do visit your gastroenterologist, if problem is not solved. You may need a repeat upper GI endoscopy.
- Some life style modifications many times become more helpful. Weight management is essential.
- Avoid spicy food.
- Take food regularly. Small but frequent meals are better than 2 heavy meal in a day.
- Include few minor meals in your diet.
- Brisk walk in morning and evening.

i have undergone several endoscopy and breath test and H-Pylori is present, the problem is that my consultant is finding it difficult to eradicate the H-pylori with the anti-biotics which are sensitive to my strain of H-PYLORI,. wHAT DO i DO STOP SALIVA BUILDING IN MY MOUTH when sleeping and the constant reflux and near choking in my sleep

Hi,

Avoid lying down after a large meal.

Eat smaller meals and maintain an upright, relaxed posture

Avoid fats, chocolate, Caffeine, Peppermint, Carbonated drinks and alcohol. Avoid smoking.
Take lots of water.

Reduce your weight.
