As per the picture, the patient is having chronic middle ear disease with tympanic membrane perforation.
Please tell me do you have any history of ear discharge?
Any hearing loss?
Any other symptoms like ringing or dizziness?
Any Investigations done like an audiometry and Ct scan temporal bone?

What medicine have you been taking so?
Follow up with the required information so that I can suggest you better.

Slight hearing loss as hear won’t pop
No dizziness or ringing
No investigation done

Meds only otc .. Zyrtec D

Thanks for the information.
The cause can be a frequent nasal infection or ear infection leading to drum involvement. The ear must dry first. you should use antibiotic ear drops three times a day for a week.
Such cases usually need surgery but it would be better to advise you anything else after I get to see the Ct pictures and audiometry.
Keep the ear dry.
Follow up with the reports.
