Hand, Shoulder, Neck Tingles And Pains, Lower Back And Chest Pain.Done Cardio Catheterization On Heart. Suggest?

Question: Hi,
I'm a 29 year old male with tingling, pain, and weakness in my left hand, shoulder, and neck. I often experience the neck and shoulder pain first and as a result my hand and forearm tingles and become weak. This has been on going for almost two years now. Recently I've experienced pain in my chest on the left side when I breathe. More so when I inhale. The pain is a sharp pain that often radiate to my left shoulder blade. I also have lower back pain and stiffness. I've recently had a cardio catheterization on my heart and the results came back negative for pulmonary hypertension. I also have a cervical MRI with no signs of a pinched nerve.
Also when I run, my shoulder pain makes me nauseous. The last three times I ran, it felt as if someone was squeezing my heart. Maybe it was muscle related but it hurt like hell. I thought I was having a heart attack. My left pec is swollen and tender to the touch. I don't know what else to do. Could you please shine some light on a possible source of my pain? Thank you for the query.

It looks like your symptoms are caused by some neural conflict. It can be compressed nerve of the spine in the lower than cervical area, spine scoliosis. Pain when breathing is quite characteristic for rib nerves neuralgia due to pine disc herniation.
It is also possible that your symptoms are caused by carpal tunnel syndrome. This condition is caused by median nerve compression in the carpal tunnel. This compression gives pain and numbness of the thumb, index, long and radial half of the ring finger. The pain can radiate to the arm and shoulder.
You can perform simple test. Flex your wrist down gently as much as possible and hold it for 60 seconds. If the numbness appears or pain, carpal tunnel it is.
To find out if the nerve is compressed and on what level, you may visit neurologist and have EMG (electromyography) done.

Heart condition should be also checked with such symptoms. ECG, 24 Holters ECG, stress test and heart ultrasound should be considered with your cardiologist.

Chest or lungs diseases can also sometimes give such symptoms. So chest CT should be also considered (chest CT and thoracic spine CT can be done together).

If this pain is muscular, you may try Diclophenac 50 mg twice a day with Metamizolum 500 mg twice a day and see if this helps with the pain. If yes, heart or lungs disease is less probable.
