Question: Greetings I have been having genitourinary symptoms over the last year. It started with urethritis, then some prostate symptoms. Went to the urologist, did urinalysis and culture, all normal, took a lot of ABs, and was followed by 6 months to conclude there was no infection. Never did any specific STI tests though. Never had fever, or discharge from penis, Urine Flux was always normal also. When I stopped being followed symptoms improved but I have recurring balanitis from time to time and in the last days I have been feeling some pelvic "fullness", balanitis again since yesterday, and some itching (not too pronounced) and diffuse/discreet pain above the penis, in the pelvic area. I did urinalisys and blood test 3 days ago and everything came back normal, pH, PCR, white cell count both in urine (0) and blood normal, no nitrites, no red blood cells in urine. Nevertheless I feel very anxious with this symptoms. What is your opinion, can I have an infection with normal parameters in blood and urine analysis, and after all this time? It's very hard convincing myself it's all anxiety with all the symptoms, they are very concrete and present...

1.most bothersome symptoms right now are a feeling of fullness, itchiness and some tenderness in the pelvic area, general discomfort in that area, not on the genitals, and it's the first time I have this, also I have some redness in the glans and meatus (this one is recurrent, comes and goes, and usually appears if I ejaculate and stays for a 2 or 3 days). If the symptoms were isolated I wouldn't bother much, but all together are causing me much concern. 2. Attached the copies of the analysis results, but they are in Portuguese. 3. I made antibiotics, azitro 1g, cefixime 400, cipro, trimetroprime and duloxetine for some time, all of that is finished. 4. My mistake, I'm a 35yo male. Thank you in advance

Hello,

Have gone through all the reports.

What you are describing are typical symptoms of
recurrent UTI, probable acute prostatitis and recurrent
balanitis.
This will need to be confirmed after a clinical examination by a
qualified Urologist. It'll be better if you consult him with the
following reports :
1. Uroflowmetry.
2. Urine culture (one week after the last antibiotic).
3. FBS, s.creatinine VDRL and HIV.
4. U/S scan -KUB with PVR.

Thank you for your answer 1.urofluxometry did in the last doctor apointment, was completely normal, according to the urologist. 2.urine culture did twice, was negative before and after AB therapy. 3. FBS and creatinine are normal in the exams I sent attached to you. VDRL and Hiv I did routine one year ago, negative, and didn't have risk beahviours since then, also had a strep throat and did a full IM penicilyn so VDRL is not justified in my case, I think. 4. Never did U/S scan because my urologist didn't think it was necessary, as he didn't find augmented prostate to the touch, or other anomalies. Also I wanted to add that the uranylisis that I sent attached is normal, so I believe even a UTI is not easy to be happening with normal urine pH, no nitrites, no leukocyte count, no proteins, etc do you agree? I'm adding this info, because all of the info and counseling you provided were asked by my urologist before to no result. I tried to feel groin lymph nodes without success also... I though the discomfort could be related to that...

Your symptoms of fullness in the lower abdomen, tenderness in the pelvic area, itchiness and with general discomfort in that area needs a Urologist consultation for a clinical examination.

A scan is indicated to evaluate causes for your symptoms other than prostate gland.
Only urine culture can rule out a UTI.
