Goal: Can You Please Render Your Opinion Of The Attached

Question: Goal: Can you please render your opinion of the attached 6 lead EKG?

Background:

1. I was complaining of a lightheaded episode around 7/17/19 - episodes that I've had for years, though frequency has varied widely from weeks to months apart.

2. On recommendation from my doctor, I wore a Holder monitor for 48 hours, which my doctor showed premature heartbeats.

3. I asked a cardiologist on this forum to review the report, which I have attached for you as well for reference. He said "However there are frequent ectopics which are usually benign and won’t cause any health issues, although in some patients they can be bothersome and annoying thereby affecting quality of life. In those cases a simple way is to add a low dose beta blocker to counter these ectopics."

4. Wanting to monitor my heart at home, I purchased a "AliveCor KardiaMobile 6L", a six lead wireless consumer device. The second document attached is the EKG output from this device today when I was feeling fine.

Goal: If you could please look at the attached EKG (generated from the consumer device), in light of the Holter monitor report (generated from a hospital device), and render your opinion?

My ultimate goal is to be able to capture an EKG if I am feeling any symptoms to be able to share with my doctor, but now I am essentially checking the appropriateness of using this device for that purpose.

I really appreciate your help.

Detailed Answer:
Hello!

Welcome to Ask a Doctor service!

I read carefully your question and reviewed carefully your reports and would explain that you should not worry too much about those ectopic heart beats, as the Holter monitor shows only isolated extrasystoles.

The amount of those premature contractions is not too much and they don't show any complex arrhythmia ( like ventricular tachycardia).

These findings are not life threatening.

Coming to this point, it is necessary to investigate for the possible cause underlying them.

First of all it is necessary to undergo a cardiac ultrasound in order to examine the cardiac structure and exclude possible cardiac structural disorder ( dilated, ischemic, hypertrophic, cardiomyopathy, etc.).

Other causes like thyroid gland dysfunction, electrolytes imbalances or anemia and the pulmonary function tests should be checked too.

If after the prolonged ECG monitoring the arrhythmia keeps persisting, then a cardiac MRI would be recommended.

Only after the above clinical investigations, betablockers could be considered.

Regarding the device, the ECG seems perfect. So, it is working properly.

Hope you will find this answer helpful!
