I am so sorry to hear about these concerns with testosterone replacement. In the strictest sense, testosterone does not cause splenic enlargement. However, it causes an increase in red blood cells which are eliminated by the spleen. This may lees to a very tiny increase in size but not enough to lead to noticeable and concerning increase in the size of the spleen.
