Peyronie's Disease PD is a curvature of the penis, particularly during erections, that is usually associated with a palpable area of fibrosis in the tunica albuginea. It is often preceded by painful erections and may be associated with ED. The curvature is usually obvious when the penis is erect but is occasionally noticeable even when the penis is flaccid. The fibrotic area, known as a plaque, can vary in firmness and sometimes becomes calcified.

If the plaque is located distally and the angulation is not very severe, a plication procedure can be offered. This operation, usually performed through a circumcising incision, is best suited for a man with a large penis. The ease of the operation and its large chance of being successful must be weighed against the almost universal resulting loss of some penile length. The amount of loss is usually approximately 1 cm.It is original amount of loss.
The mean loss of penile length was 0.36 ± 0.5 cm with a range of 0-2.5 cm. The authors identified 3 factors that were associated with greater postoperative loss of penile length: larger preoperative penile length, greater preoperative curvature, and a ventrally placed curvature.

When the penile length or the patient's preference precludes the use of penile plication and if the patient retains erectile ability, then an incision or excision of the plaque can be considered with grafting to reconstruct the tunica albuginea. In this case no or very less amount of loss of penile length.

Normal size of penis in flaccid state is 2 to 4 inches while in erect position3 to 6 inches. It varies according to body structure and race. Size of penis does not matter much for satisfying partner.
Anterior 2-3 inches of female vaginal wall is sensitive to sexual stimuli. Only 3 inches of erect penis is sufficient to satisfy female.

So even after losing penis length if it is more than 3 inches, no issue of erectile dysfunction.

However there is other aspect also. Degree of curvature and duration of PD also matter in prognosis for erectile dysfunction. If more than 60 degree and more than 6 months of duration there is more fibrosis and more chances of nerve problem and erectile dysfunction.

Loss of 7 cm of penile length is not regular after surgery. May be in rare individual case it had happened. With recent advances in surgical techniques not more than 3 cm lose is there generally.
You became apprehensive likely after reading some unauthentic information.
And in any case if shortening is there it can be lengthen using circular venous grafting and daily stretching with a vacuum erection device.

The etiology of PD is not yet confirmed and remains enigmatic. It has been associated with deficiency in vitamin E, ingestion of beta-blocking agents, and elevations of serotonin levels. PD is associated with Dupuytren contractures and with HLA-B7, implying a genetic link to its etiology.

More recently, PD has been thought to result from vascular trauma or injury to the penis. The injury may be trivial or involve only microscopic vessels and tissues. This triggers the release of cytokines that activate fibroblast proliferation and produce collagen, the main matrix component of a Peyronie plaque, within the tunica albuginea. However it does not mean that it happens due to normal sex. Sex only precipitates the situation or can reveal PD. And if by chance we think that injuries during sex can cause such problem it is only in whom who are prone to PD and that is also not confirmed at all.
