Does Condom Offer 100% Protection Against HIV And Other STDs?

Question: Hello
I have few questions regarding STDs

I am 38 year old male and I have what you may call a higher risk lifestyle , I like having sex with higher payed prostitutes ( I do not use cheep $ 10-20 hookers ) My range is typically between $ 200 and $ 400
I always use a condom for both oral sex ( receiving ) and for vaginal sex . I know I have Herpes 1 & 2
also HPV for many years but no HIV. I really enjoy performing oral sex on prostitutes ( no protection )
and french kissing my date .

I am trying to determine my risk level for STD's , I do routine std check luckily I have tested negative for Chlamydiae ,gonorrhea ,syphilis and HIV . But certainly Std's are always in the back of my mind since I am
a married man . So this was just a brief introduction to me . now here are my concerns:

A:
Lots of doctors say that if I use condoms ( Ones I use are Trojan thintensity ) for oral and especially vaginal sex chances of contracting Chlamydiae ,gonorrhea , Trichomoniasis and HIV are nil .
I have heard recently there was a study that condoms are 80 % effective against HIV and even
if you use a condom properly each time you can get infected . I am confused who is right .....
Have you ever seen cases of 2 straight partners one HIV pos ( Female ) and one HIV neg ( Male ) getting infected through a condom ?

B:
How is my choice of condoms ( Trojan thintensity ) ? I enjoy the thin kind they never failed me yet but should I switch to thicker condoms for better protection ?

C:
Since I have Herpes 1&2 should I worry about acquiring some new herpes super strain ?
( I recently got diagnosed with herpes a year ago so I am not sure how long I had them ,
I never get breakouts ) If I acquire new strain would I start getting break outs ?

D:
As far as performing oral sex on a girl am I in any risk of catching something ?

E:
How common is gonorrhea of the throat ? Is this something to worry about ?

ohh last question.

If a person gets chlamydia or gonorrhea or Trichomoniasis can they
just live with it without treatment ( at least for a year or so ) ?
will this be painful ?
My wife and I are not planning to have any more kids so
I'm just trying to cover all my bases here .

I have gone through your query in detail and I have noted down your concerns

A) As far as protected sex is concerned it is protective against HIV except in rare instances when the condom breaks Or ruptures.
Therefore a condom, if used correctly is almost 100% protective against HIV. It also completely protects against gonorrhea, chlamydia and trichomoniasis.

However, a condom does not protect against all STD's. STD's like herpes, HPV and syphilis can still be transmitted by condom protected sex.
This is because a condom covers only a portion of shaft of penis and therefore leaves a lot of neighboring skin exposed for contact with infected genital skin and secretions.
Therefore, base of scrotum, base of shaft of penis and adjacent pubic skin can manifest with genital herpes, syphilitic chancre and HPV warts inspite of a condom protected sex.


B) A thin condom is good as far as protection against HIV/ STD is concerned and a thick condom does not offer any greater protection as compared to a thin one.

C) There are only 2 strains of Herpes i.e type 1 & type 2.
However, already being diagnosed with herpes infection does not protect against re-infection with the virus.
Therefore, sexual contact with a herpes infected partner might lead to re-infection and a clinical episode of herpes.
Moreover you can transmit herpes to your sexual partners even when you don't have a XXXXXXX clinical episode as the virus is being shed asymptomatically from time to time.

D) Oral sex is not completely safe although it is a lesser risk as compared to vaginal and anal sex.
Almost all STD's like herpes, Hiv, Syphilis, throat gonorrhea and chlamydia gets transmitted through oral sex if your sexual partner is infected or has oral lesions.

E) Throat/ pharyngeal gonorrhea is a definite risk through Oral sex.
The exact incidence is not known but it is fairly common. Symptoms would be the same as that of a sore throat.

F) Urethritis due to chlamydia, gonnorrhea, trichomoniasis would usually be symptomatic in a male i.e urethral discharge, burning sensation on passing urine, urethral pain etc and therefore infected males usually seek treatment for urehritis due to these causes.
Unprotected sex with your wife poses a risk for transmitting STDs like herpes, HPV to her.

Let me ask you Doctor.

As far as Throat/ pharyngeal gonorrhea or oral Chlamydia, if I perform oral sex on a sex worker and I become infected . I then have unprotected oral sex with my wife
she gets infection in her vagina and I get infected through having intercourse
with my wife ? Is this how this scenario works ?

Also I have heard that Throat/ pharyngeal gonorrhea or oral Chlamydia
does not stay in your throat very long , it will simply go away on its own
because those germs do not like that kind of environment , can you clarify this for me ?

Pharyngeal gonorrhea is self imited but a risk for genital infection

Detailed Answer:
Hi.

Pharyngeal gonorrhea may be self limited.
This, however, does not eliminate the importance of pharyngeal gonorrhea as a source of genital infection, because oral sex has been shown to be an independent risk factor for genital gonorrhea.
Therefore you infecting your wife through oral sex and then she giving it back to you is a possibility.

Vaccination is only available for HPV particularly against 4 strains of HPV i.e type 6, 11, 16, 18.
Hpv vaccine prevents infection only with these HPV types and not all HPV types. Moreover, it does not cure those already infected.
More than 90% of genital warts are caused by type 6 and type 11.
The ideal time to get vaccinated is before one ventures into sexual activity. Since you are already infected with Hpv therefore I don't find any benefit of Hpv vaccination in you.

There is no vaccine for rest of the STD's.
