Does Bariatric Surgery Help In Weight Loss?

Question: I am conteplating Bariatric Surgery
I am 5'10 274 pounds and am a classic metabolic syndrome patient. I have type 2 Diabetes and only on insulin because my CKD stage is 3. Was on Metformin also.
All of the following I take medicine for:
Triglycerides - 800
BP- controlled on 5 drugs
Type 2 Diabetes- not well controlled
OSA - with cpap controlled
NASH- dr stopped tricor, but on Crestor & Lovaza
Low T - untreated
Right sided HF with normal EF.

Bottom line is I have gained alot of weight over last 4-5 years. 20 in last 2 years
I am religious with my drugs and Cpap machine. I am not with exercise or eating habits. Not huge junkfood, but eat out alot and I travel alot.
I am told by bariatric surgeon that I am the textbook patient for gastric bypass, especially since in many people Diabetes vanishes for some reason, prior to even any weight loss. In a perfect world, with me keeping a revised lifestyle at the forefront, diabetes gone and dramatic weight loss will lower BP, Trigs, etc, etc

I am starting to lean towards the sleeve instead,
-less risky surgery
-pretty close in overall weight loss
-no nutrition issues/hairloss etc.
-less recovery time
My biggest concern is the Diabetes beating on my body and especially my kidneys. I figure if I lose 40+ pounds with sleeve surgery my A1C and Triglycerides should go down as well as the fat in my liver? I have been taught though that if u have Type 2 Diabetes, even if u get your A1C normal, Diabetes does not technically go away.

I am only 50 years old. I need to make the best decision based on my current health, but realistic what I will do with my lifestyle, especially with my sales traveling and eating out too much.

Do not believe have been tested for c peptide, but here are some other hormonal tests I have had.
