Does Stress Tablet Have Any Effect On Urine Analysis Test Results?

Question: Dear Doctor,
Im a male. I received unprotected oral sex from a female co-worker 2 years back. After that no any sexual exposure. After 2 years suddenly i thought of STD transmission through that exposure. I was looking to test for Chlamydia and Gonorrhea. One of my friend he is a general doctor told me to do Urine Analysis Test. I did and there is no WBCs in Urine. He said no infection present. My worries are
1) Since i tested 2 years after my exposure for WBCs, will the Urine Analysis test will be 100% accurate that there is no infection present?
2) I was taking Stress tablet for 5 days during the test. Will that tablet will affect Urine Analysis test? and WBCs will not show in test.
3) After this doubt i was searching in google. In google every where it says only PCR test should be done for Chlamydia & Gonorrhea but not Urine Analysis Test.

Please help me with your answers.

I am dr.kakkar(dermatologist and venereologist).

Gonorrhea and chlamydia are usually symptomatic in males. Usual symptoms are urethral discharge and burning micturation. However since you dont have nor ever had any such symptoms therefore it is unlikely that you have the infection. Of course PCR is more accurate and specific as compared to urine analysis but there is no indication to test for these infections now after 2 years specially since you dont have any symptoms. Moreover oral sex carries negligible risk for transmission of these STDs.

Dear Doctor,

Thank you for your reply. Since i was filled with anxiety i just want to make sure that im not infected.
1) Do you think that Urine Analysis test is accurate after 2 years?
2) If infection present now that is after 2 years of exposure will the Urine Analysis test still have WBC in urine? I know it is not specific.

Is there any risk for STDs(Hiv or Hepatitis or VD) for babies through saliva from hand to babies mouth? My neighbour lady fed my baby a tiny piece of snacks through her fingers. This lady is risky lady. So im worried of my baby catching any STD through her hand that is if there is saliva or blood present in her hand when she feed my baby.

Hi.

No. Urine test is not at all accurate for gonorrhea or chlamydia. It is only suggestive for non gonococcal urethritis i.e chlamydia

There is no risk of Hiv through saliva. Hiv is only transmitted through either sexual contact, injection in drug abusers who share needle, transfusion of infected blood or through breast feeding in neonates.

Since im in anxiety i just want to test for Chlamydia and Gonorrhea.Please suggest me the reliable test and accurate test for Chlamydia and Gonorrhea. My exposure was 2 years back so plz suggest me test accordingly. Also mention the specimen whether the test to be done in Urine or Blood.

Will hepatitis B&C transmitted through saliva?

You may take a urine NAAT test (nucleic acid amplification test) for chlamydial or gonococcal infection. However, it would most likely turn up as negative.
No. There is no risk of transmission of hepatitis B or C through saliva.

I read in internet that after few months Chlamydia & Gonorrhea will spread into blood. Is it true? Since my exposure was 2 years back may i have chance to spread into blood? If so the same Urine NAAT test will be sufficient to find Chlamydia and Gonorrhea infection? or we have to find through blood test? If yes for blood test what blood test is there for Chlamydia and Gonorrhea.

Hi.

Urine test is sufficient to detect these infections. Blood dissemination is usually associated with symptoms like fever, joint pain and a skin rash. It is not asymptomatic. Therefore i wont keep this possibility in you.

Thanks for your reply. I understand that since im asymptomatic i no need to worry.

1)But what is the test for Chlamydia and Gonorrhea if it enters into blood. I want to know this detail. Plz tell me what test for Chlamydia and what test for Gonorrhea if it enters into blood.

2)Is there any chance of STDs transmission from hand to mouth for babies? If there is any small amount of Saliva or blood present in fingers?
3) If my Blood Sugar is 350 will it affect any STD testing like HIV, Hepatitis B, Hepatitis C and VDRL test.
4) For testing Hepatitis B, HBSAG test is sufficient or we have to Anti -HBC also?

Hi.

Blood culture and joint culture can be done to detect systemic dissemination of chlamydia and gonorrhea.

Hsv can transmit by indirect transmission through hands to mouth in babies.

Blood sugar does not affect STD testing.

HBsAg is sufficient for HBV, if you have never been immunised by Hepatitis B vaccine in the past.

Thanks for your valuable reply and patience in answering my questions. Plz answer my following and i hope it is my last questions

1) If Chlamydia and Gonorrhea infections are transmitted to blood, still they will show in urine NAAT test or it will not show up in urine test and it will show in blood test only?
2) I have read in internet that few HBV infections will be present in the absence of HBSAG? Is it true? If true When does this condition happen? At this condition what are other test to find HBV is present or absent?
3) You have mentioned that unless im immunised by Hepatitis B vaccine you can just rely on HBSAg alone for HBV infection. For immunised persons what are all the test to find HBV infection other than HBSAG.
4) VDRL test always shows positive once the infection is present or after some years of infection the test will not show positive even if the infection is present?
5) After taking alcohol how many hours or how many days we should test for STDs?

Hi.

1) These infections can be detected in urine as well. However, blood culture is to confirm the systemic disssemination with these organisms.
2) In fact HBsAg indicates either acute or chronic infection with HBV and is the most sensitive marker of infection.
3)I want to correct myself - I meant if you are immunised you would have anti-HBs anibody (not HBsAg, as I said earlier). In fact presence of HBsAg always indicates infection. HBsAg remains the most sensitive test to determine infection with HBV.
HBsAg negative hepatitis occur in rare settings like:
-the person may be recovering from acute HBV infection Or
-may be distantly immune, but the test may not be sensitive enough to detect a very low level of anti‑HBs in serum Or
-may be susceptible with a false positive anti‑HBc Or
-may be chronically infected and have an undetectable level of HBsAg present in the serum
4) VDRL may become negative after years of infection.
5)Alcohol has no interaction with STD testing.
