Posted on Sat, 16 Aug 2014
Question: I am diagonised with Gastritis with the help of Endoscopy and blood test. I started on Pantocid HP kit and would like to know whether it has any adverse side effects and what would relieve those side effects. Also can you suggest fruits that I should be eating through out my medicinal course that will not only control the side effects but also help me healing quickly?

Detailed Answer:

Hi XXXXXXX
thanks for posting your concern in the HCM.

I would answer your questions step by step.

1. Pantocid HP kit contains Pantoprazole 40mg, Amoxicillin 750 mg and Clarithromycin 500 mg. The first one suppresses your acid secretion in the stomach and the later two would kill an organism called H. pylori that causes gastritis and peptic ulcer disease.
These medications are usually safe unless you are allergic to one of them.

However, every medication has its side effects. The most common side effects are metallic test, pain abdomen, altered bowel habit including diarrhoea, dyspepsia, headache, nausea. The acid related side effects of clarithromycin will however be suppressed by pantoprazole.
Some very rare but serious side effects incude neuropsychiatric problem, QT prolongation leading to bradycardia, pancreatitis, liver dysfunction etc which occur in less than 1%.

2. Consumption of fruits, particularly citrus fruits, will give your body the strength to fight against ill effects of medicines because they will provide you vitamins and antioxidants that will protect your own cells. But, I don't think the commoner and temporary side effects like altered taste or headache or dyspepsia can be prevented by fruits, neither will they prevent cardiac arrhythmia or pancreatitis etc. But, consumption of 100 gm fruit per day especially orange, lemon, grapes, XXXXXXX pomegranates, banana will be very good for your health.

Hope you are satisfied with my response. If so, please rate my answer. If you have any further questions please write back to me.

Thank You very much Dr. XXXXXXX for your reply, however I have few follow up questions:
1. What about Apple? Especially during the Pantocid treatment period?
2. Even before Gastritis I used to get occassional headaches (due to stress, workload etc.) which I typically get rid of using Flexon tablet. Is it safe to take Flexon tablet during the treatment period? If not can you suggest an alternative safe medicine for headache?
3. After 10 to 15 mins of taking the Pantocid dosage, I feel severely hungry. Typically when I am severly hunger my hands and legs starts shaking. Is this a normal symptom? or anything to worry about?
4. Is there any particular diet that I need to cut down on my daily meals? E.g. I generally prefer sambar/rasam (Indian variation of tomato soup) whenever I am Ill. I also prefer pickles, and my doc said no issues with pickles though he did not encouraged. What do you suggest?
5. Will having Enama be of any help in improving the recovery of Gastritis?
6. There is a common notion about Citrus fruits not to be taken during Gastritis and I felt it as a myth than a truth. Though you mentioned Citrus fruit in your list, I want to confirm once again
7. Should I cut down on Tea?

Best Regards,
XXXX

Detailed Answer:
Thanks XXXXXXX for your prompt reply.
1. Apple is a wonderful fruit, rich in antioxidants, Vit. C, B complex and beta carotene. It will be very good for your body, especially during the treatment period.
2. I won't advise you to have any analgesic for your headaches. You already have gastritis. Although you have H. pylori, analgesics are important precipitating factor for gastritis and if taken furthermore, it may cause an ulcer. Please consult a neurologist for evaluation, if headache becomes unbearable. However, if it is stress headache, you may have relaxation sessions to get rid of that.
3. Increased appetite may be associated with pantoprazole intake. But, it is also caused by certain endocrine problems, e.g. diabetes, thyroid disorder, mental disorder e.g. depression, bipolar disorder etc . First, you need to exclude whether it is increased hunger or "hunger pang". To my understanding, it is hunger pang which is a symptom of the disease you have. But, you can definitely consult your doctor to rule out any underlying pathology.
4. Tomatoes are very good for health too. But, I won't advice you to have pickles, especially when you have gastritis. In addition, spices, smoking( if applicable), alcohol (if applicable) and carbonated beverages should be avoided. Sambar can be taken.
5. Are you talking about "enema"? If yes, I don't think that can help. They should only be given if you have refractory habitual constipation and that too after your doctor's advice. But, you can have ispagulah husk at bedtime. That is definitely good for the gut.
6. And that notion about citrus fruit is because people think the ascorbic acid can cause more acidity that would affect the stomach. But, that is definitely wrong. They are actually very good for health.
7. Excess tea is not good. That contains caffeine, which can aggravate your gastritis. But, you can have it once or twice a day if you want, because the amount of caffeine is very less in it.
