But as cea not dropping does it mean it not working and more mets forming?

My dad has 1 liver met 16 mm n 1 aortocaval node. Primary tumor in colon was removed in march last year.
Cea was 9.8 august, 8 oct, 9 dec and 10 january

CEA in your case seems to be on increasing note.

This suggests possible new mets rather than in capable drug.

Its highly recommended that your father undergoes a total body PET scan at the earliest to document the location of new growths if any along with a complete study of the solitary met with one aortocaval node.

Also Solitary liver met can be safely resected in most cases to prevent progression/relapse of the underlying cancerous process.

Yes such a possibility exists.

Yes there is a possibility of developing new mets even while being on therapy with Irinotecan,if the tumor cells undergo/acquire new spontaneous mutations.

Irinotecan even though a highly effective drug ,will not completely protect against newly mutated cells.

This report implies extension of colonic cancer beyond the wall of the intestine outside the muscle coat.

From inside to outside the layers in the colon are named as 1,2,3,4.

Muscle coat is layer 3.

Cancer begins in layer 1 and spreads outside to involve 2,3,4.

Extramural implies the location of cancer between layers 3 and 4.
