Does IBS Causes Constipation?

Question: Dear Doctor
Its been a month since I faced an episode of constipation. I also lost a lot of weight . I used to weigh 95 kgs , now it's 86 kg. I'm 6 ft 2 inch tall guy.

When the constipation occurred , I pushed the bowel hard which caused some discomfort in lower back (area above buttocks in the line parallel to ground).
I did a tremendous lifestyle change
eating fiber diet ( cereals and papaya for breakfast , louki and spinach for lunch and dinner and louki soup just before dinner. I also started a lot of walking everyday.D drinking lukewarm water every morning with honey etc.
My stools were normal but I still faced mild pain around both sides of belly button. I also faced stiffness at times in lower back and faced a sensation of gas travelling up my spine and head causing discomfort.
When I travel to different city , constant sitting causes air lock and until I walk the gas doesn't seem to pass.I feel bloated and the constant gas formation causes issues.
I got my blood tests , heart test , ech done etc. everything came normal
I do not understand what is happening here

It could be Irritable bowel syndrome (IBS).

As per your description, you are very health conscious.That's good.
Too much of consciousness about abdomen will raise lots of doubts.It leads to IBS too.
Gas accumulation inside the bowel is due to the food that you have taken and also due to air that has gone inside during swallowing. During peristalsis, bowel movement will occur leading to passage of flatus. In some,movements will be slow .
If ultrasound abdomen is also normal,then the symptoms go more in favour of IBS.
You should reduce your anxiety regarding gas formation. I prescribe a prokinetic agent like Itopride 150mg once daily, Pre and Probiotic capsule twice daily after food to my patients with similar complaints.
Please do regular exercises. You can start with Walking daily.
Nothing to worry.

Hello Doctor
I haven't done Ultrasound for abdomen yet. I actually have pain sensations around the belly button even when the flatulence has subsided owing to belching or farting. Although stools that I see are normal . Also when tightness is experienced , the motion stops and I have to walk , drink hot water etc. to relieve myself from that situation temporarily.
Is this also indicative of IBS ?

You should get an Ultrasound abdomen.

Hi
You should get an Ultrasound abdomen.Appendicular dyspepsia can cause these symptoms.
If there is no definitive pathology in Ultrasound, IBS can be considered. The symptoms that you have mentioned go in favour of Constipation dominated IBS.

Dear Doctor
I'll get it done this week. As of now on the suggestion of local gastroenterologist I did went for stool test (report attached ). He also asked me to take unienzyme and esogress D and an anti depressant for 10 days.
I just took it for a day and experienced cramps and pain in lower back and around waist area. I believe they are aiding in constipation although they reduced flatulence a bit but it didn't help much.

Shall I go for some milder tablets that can help kill intestinal bacteria growth causing flatulence ?
Please suggest
Also suggest me some therapy based exercise if it is helpful in reducing the muscle cramps around the belly , Lower back and waistline

Hi,

It is not appropriate to kill intestinal bacteria. Flatulence is not due to bacteria. You should take a prokinetic tablet along with Pre and probiotic for symptomatic relief.
For exercises, you should consult a qualified physiotherapist.
