Question: I have Gerd. recently when I lay down at night I get horrible burning pain in the middle of my back. I have to get up. Previously the burning pain was always in my chest. this has been happening for the past six nights. I have to sleep in a chair sitting up. What is happening?

* As per my surgical experience, the severity of pain indicate underlying inflammation of
substantial amount involving stomach, duodenum, intestine, pancreas, gall bladder or
else.
If it was simple gastritis, there should not be this severe agony of pain.

Kindly provide me further details to proceed further with the query.

1. Pain happens only when laying down at night or also appears during day ?
2. Associated vomiting, alteration in bowel movements if any.
3. Any significant changes in dinner item particularly before the onset.
4. If available and possible, kindly upload reports in follow up.

1. During the day I have a gnawing pain just below my left rib cage. the worst pain is at night. Can not sleep. Woke me up 6 nights ago and has not stopped or changed even with the meds I am taking. It is helping the burning in my stomach during the day. but the pain is keeping me from my normal activity. It hurts to wear a bra because it causes pain in the center of my chest.
2. no vomiting, or change in bowels.
3. I ate crab legs the day before this started. I only eat them once or 2 times a year.
4. I can not upload reports. The radiologist reported the CT scan to be no acute findings in the abdomen or pelvis. The Dr. said all my blood work up was good. no findings.
I have never experienced this radiating pain in the center of my back. Like I stated, my discomfort from GERD caused the burning pain in my chest. Not this time.

Thanks for the details.

* As the CT findings are not conclusive, there must be inner mucosa issue which can not be detected many times on CT scan as acute gastritis, duodenitis ( posterior wall ulcers are more likely to give severe radiating pain to back ) or else ; the most likely cause is consumption of crab legs.

* My recommendations at present

- Drink more liquids in sessions.
- Consume bland, vegan diet with organic fresh fruits.
- Addition of dicyclomine with paracetamol tablet will relief pain to much better extent.
- Continue rest of the medicines.
- Discuss about the need of upper GI endoscopy with your treating physician.

Dicyclomine is a prescription drug.
