Question: Dear Dr. Spaho. You previously have been very helpful in connection with my family member’s spinal surgery, and I return to ask your advice on a new problem.

He now has a worsening problem of painful and deep jabs like needle pricks appearing at random times in both legs from the thighs downward including the feet bottoms. I am asking if you have any suggestions on what he might try given the information below.

Following surgery, the first indication of an issue of this kind appeared with tingling in the lower legs, which started about 5 weeks after surgery.

He can move muscles in one leg; but still has virtually no control over the second leg. It is in this second leg that the jabs are particularly bad.

He is currently medicated with Gabapentin-pms 300mg and Nise 100mg. Unfortunately, he gets no relief from these medications.

Hello again and thanks for the direct question.

Since there are new symptoms appearing after spinal surgery, and there was no spinal fusion, in my opinion, instability of the spine should be considered and evaluated further.

Recidivating the spinal tumor also is another possible explanation in this case.

In my opinion, a new imaging evaluation of these possible explanations is necessary.

A spinal cor MRI with contrast medium and dynamic x-ray films of the thoracic-lumbar spine is necessary for a correct diagnosis.

Discuss with the surgeon these issues.

Yours is an extremely helpful reply, Dr. Spaho. The surgeon has previously asked to be made aware of advice we are getting from you, and so I am sure he will find your observations very helpful. I guess we now need special prayers for our relative, as it seems he is getting into even deeper water he was before.

I expect that I will have a follow-up question after I share your observations.
