Question: Dear Dr Misra,

Thanks for your advice to my earlier thread

While I am awaiting for next check up, I feel that my operated right eye’ vision quality has declined a bit recently. The image is a little bit blur, similar to like myopia has worsened slightly. Could it be due to worsening cataract (in end Sep my doctor said I have NS trace).

My right eye ERM operation was done in end May. NS trace was found in late Sep and my doctor said was earlier than expected. Could you tell what is my likely pace of progression of my cataract based on your clinical experience?

I know when I go for cataract surgery, I need to determine the myopia power I want to retain. I hope I do not need to wear glasses for social distance (is it called intermediate distance?) and for reading XXXXXXX phone, but do not mind wearing progressive glasses for distant vision and reading fine prints. I am thinking of mini-monovision or both eyes (say dominant eye corrected to emmetropia and nondominant eye -1.00D) or both eyes corrected to -1.00 D (so that I could see well for social distance and OK to read my XXXXXXX phone). Any views on these two options? Or other advice? I am still working and need to read computer and read documents. I want to have early trial with contact lenses to mimick these two options so that I could have a feel of what my vision will be, to facilitate my decision when I need to go ahead for cataract surgery.

Besides, what type of IOL is recommended? My doctor advised to have monofocal ones given my eye conditions. I want to have anti-blue ray function as well, does anti-blue ray IOL make vision dimmer and with lower quality?

Appreciate your expert advice. Thanks much.

Brief Answer:
Post ERM cataract

Detailed Answer:
My dear,
Thanks for getting back to me.
Though erm surgery in May and na traces in September is a bit early but it's there and documented.Vitreous surgery leads to cataract sooner or later.I have observed that younger patients develop cataract early and it's progress is more rapid.
As a rule we prefer to make you emetropic for distance and a small power for reading is welcome.
A monofocal, foldable, aspheric lens is my first choice.Blue ray lenses do not dim the vision, they on the contrary make colour a bit more natural.
Cataract surgery at some stage has to be undertaken,so some planning regarding this is welcome.
Your idea about keeping the dominant eye emetropic and other eye slightly myopic is good and should be followed. These changes are done by us on patient to patient basis.
Any further query in this regard, please do get back to me.
