Question: Dear Dr.,

I am seeking a vascular medicine specialist’s interpretation of the results of my Doppler test, as well as her or his guidance concerning the next actions that I should take in connection with the problem I am experiencing at my lower legs and feet.

Attached you will find the following supporting materials: two photographs of the lower legs, the laboratory report on the ‘veins and arteries’ Doppler test results, a new compression stocking prescription provided by a general family physician (not a vascular medicine specialist) . I am already wearing compression stockings; but my family physician thinks they are not tight enough.

My goal is to ensure that I will have independent mobility in the years ahead by achieving adequate blood circulation in my legs and feet, and restoring normal skin colour in those lower extremities if that is possible.

Already, the affected skin is significantly rougher than normal surface and a healing substance such as aloe vera juice penetrates that area much more slowly than it penetrates elsewhere in my skin. It is essential that this condition does not spread in future months or years. And so I need your expert help.

You have poor fluid drainage from legs.

Detailed Answer:
HI,

Based on the report and images, you have poor fluid drainage from the legs. This is because the veins responsible for carrying blood back to the heart are "weak" or incompetent and thus allows blood to stagnate. This causes the leg to swell and also explains the change in skin color. This is often accompanied by dull achy pain/pressure in the lower limb. The worry is that this can lead to wounds with poor healing potential.

The good news is that your doctor recommendation fo compressive stockings is reasonable. Also, elevating the legs when sleeping would help return fluid back to the legs. If these measures do not help, you may need to see a vascular surgeon or vein specialist who can actually treat these vessels and prevent them from causing back flow of blood that stagnates in the leg.

In all, continue compressive stockings and keep legs raised as often as possible. If these do not help, see a vein specialist for procedure called ablation which consists in obliterating these veins to avoid further blood pooling, In interim, avoid any cuts/wounds on these legs as healing would be quite difficult.

Thanks very much for your important advice dr. Ditah. I am proceeding now with my local doctor in search of a specialist who is skilled in the vein-related procedure you have described. I’ve also noted carefully your remark concerning avoiding cuts.

I do have an additional two-part question for you. Is there any non-surgical vein strengthening practice that I might try? In this connection, do you know of any food(s) might be particularly helpful?

Hi and thanks for reaching out.

Lifestyle measures such as avoiding excess salt intake, raising legs while sitting, applying compressive stockings among others. I do not know your weight but weight loss and regular physical activity can be of great help in this regards. I know these may be difficult but they are worth trying/

I do not know of any particular foods that have been proven helpful. However, you will never get it wrong by eating healthy and exercising often (recommended is 150minute oof moderate activity weekly).
