Question: Dear Doctor, please suggest me what to do next, as this situation requires urgent attention. I went for a massage yesterday and the female therapist offered me to touch her breasts. I fondled with the naked breasts for sometime, and then without washing my hand I masturbated myself. Just that before masturbation i applied oil on my hand and penis. I'm very worried now, is this a risk for Hiv or common STDs if the therapist's breast milk came in contact with my hand or fingers? Should i go for Pep as only 24 hours have passed? I did not allow her to touch me and we did not have any kind of oral, vaginal or anal sex.

Thank you Doctor. This did indeed relieve my concerns and I wont go for PeP. I forgot to mention that I'd minor peeled off skin / cuts near the nails of my fingers.
1. Would this change your previous assessment?
2. Although the status of the therapist is unknown, would your advice change if she was found to be Hiv+ ?
3. I test for Stds regularly, including HIV. All come out negative. Considering this incident, can I have unprotected sex with my partner without any worry of infecting her?

Thank you for the reply. I'm trying to rationalize my thoughts. So request your help in clarifying few things:
1. Am I not at risk at all because I did not have unprotected sex (oral/vaginal/anal)? Do the medical practitioners not classify this as a risk for testing because it did not meet the criteria of unprotected sex, and additional risks like use of unsterilised needles, mother to new born (during birth or breastfeeding), all of which have not taken place. Therefore, I am not at risk and dont need to test..
2. Will this activity be classified as masturbation with bodily fluids or just masturbation? If bodily fluids are involved in masturbation does this put me at risk?
3. I used oil for masturbation, after touching the breasts. Would using the oil deactivate the virus instantly. Or would exposure to air deactivate it.
4. On internet there are many cases of people who have sucked or licked the breast, and consumed drops of breastmilk. Would this be risky? Although I have not done that, just for comparison of risk does that put me in safe zone as I did not suck or lick the breast.
5. Have there been cases were people have been found at risk because they touched breastmilk which could be infected?
6. Is fingering considered as a risk?

In summary, the 2 things i found risky were:
1. Touching the breasts and therefore breastmilk. This breastmilk touching the skin of my finger would find its way through micro tears or cuts into the bloodstream.
2. Using the same hand to masturbate myself. Thus the breastmilk coming in contact with my penis thus further going through the micro tears or cuts or urethra into the bloodstream.
I'm struggling to understand if these two qualify as risks.
