Question: Hi. I have been suffering from urine infection since a long time. I have got the urine analysis and culture done more than 4, 5 times and also scanning but nothing was traceable. I have taken many a time on
the advise of doctor(s) various Antibiotics i.e. NORFLOX, BACTRIM D ETC. WITH OUT ANY RELIEF.
I GET PAIN IN THE TESTES ALTERNATELY AS IF THOSE ARE BEING CRUSHED AND BUR'NING FEELING, ONCE I HAD SUFFFERED FROM BODY LICE, COULD THAT BE THE REASON. DOES IT EFFECT ONE's sex life also. please help me. I AM A MALE OF 61 YEARS AND INCIDENTALLY DIABETIC ALSO. BUT SUFFERING FROM URINE INFECTION MANY YEARS BEFORE BECOMING DIABETIC. MY HEIGHT IS ABOUT 5FT 11 INCHES AND WEIGH ABOUT 83 KGS. BEST REGARDS

I've gone through your query and understand your concerns,

Well, from your description it seems like an ongoing complaint for quite a while...

Let me know what all investigations have been done so far apart from urine routine and culture, please upload some of the latest investigations which have been done recently...Did you mean CT-scan or USG scan? Kindly upload the printed report of it also...

To some extent, I'd agree with the Urologist, as in case of multidrug-resistant UTI some symptoms can be persistent, but it's important to know the causative organism in the first place to decide the further course of action...

It might not be bacterial but can also be some other organisms like fungal infection too which is extremely difficult to diagnose, and treatment is not always that much effective either...it's also important to rule out any tuberculosis of the genito-urinary tract which can present in a similar way, to diagnose this, apart from scanning and urine routine culture, some other investigations need to be performed...

I'd like to know if any investigations performed to rule out STDs so far...

Although as you've mentioned the complaint persisted prior to having diabetes, please note that an ongoing Diabetes can complicate it further, thus making it even more difficult to treat...So, please make sure the blood sugar is strictly monitored and controlled as per your doctor's advice along with strict lifestyle and dietary modifications...

The body lice don't seem to be related to this...Diabetes, as well as infection, can affect sex-life in the long run also...

It's definitely a complicated scenario...However, please upload/attach those reports and the latest prescriptions of your Urologist so that I can look into it more specifically and assist you further...

Alternatively, you may suggest any further investigations I may have to take, so as to diagnose the problem correctly. I guess that you are from XXXXXXX and if correct pleace give your clinic address or XXXXXXX so that I can catch up with you in the first week of next month during my visit.

Unnecessary investigations can only add on to further confusion, so let me know if at all any evaluation done presently and what's the last prescription/medications are you following....No, I'm not, I'd suggest you be under strict treatment and supervision of your Urologist only for further management, it's always ideal to be under treatment of the Specialist who's already handling your scenario and following up with you routinely...
