Can Muscle Spasms In Both Sides Of Pelvis Cause Pain While Sitting?

Question: Hey Dr. Shanthi.E its XXXXXXX how are you?
Thank you for checking up on me. You are so sweet:)
Well I followed up with another gyno with more experience and she thinks it is muscle spasms that is causing the issues. She thinks on the right side where the cyst got broken with the first nurse practitioner doing the pelvic exam caused the muscles around the ovary to go into spasm and that is why it hurts when I sit up or bend over but not when i stand or lay down.
On the left side where I started to get the pulled muscle cramp feelings after the transvaginal ultrasound she thinks same thing, when she was moving it around inside she caused a muscle down there to spasm which might be entrapping a nerve.
So she wants me to get a pelvic mri with contrast and a lumbar mri to rule out there isnt anything else going on cause she said the lumbar could be part of the reason for spasms as the muscles and nerves are all close and the fact when I bend the pain gets worse on right ovary, could be something with back muscles even though tightenss is in front by ovary, not back, and she talked about possibly seeing a chiropracter for adjustment if they find anything in lumbar sacral mri. If back ok then she said pelvic physical theropy would be next step.
I am not big on chiropractors or adjustments so I think regardless of mri of back (I havent hurt my lower back before or since this all happened so why would it suddenly be from lower back right or ?)
I am more a fan of trying the pelvic physcial theropy first i think.
I had pelvic PT 4 years ago when I went to a different gyno for my yearly check up and she did the speculum way to tight that I got this constant pee feeling after and went to a bunch of urologists but the urine test always came back negative for an infection. Finally after a month of suffering someone recommended PT down there and this woman, XXXXXXX Freedman, basically gave me my life back!!!
She is amazing and new how to push on certain muscles inside the vagina to loosen them up and get rid of the spasm I had on the one side that was causing me to feel like i always had to pee, even when I didnt!
I am a little worried about going back for the spasms I appear to have now as I dont have that pee feeling issue and I am scared it might come back with her having to go in there again, might re-set it off but in the end if it gets rid of all the other spasms, who cares, I need my life back!!!
So that is where things are at for now. What are your thoughts on all of this? Anything to add?

I am doing good; thanks for writing in.

I do agree with the other Gyno that it could be the muscle spasms on both the sides of the pelvis.

You can get a pelvic MRI with contrast. That would be good to find out the cause of your symptoms. a lumbar MRI as such is not required now. However, if it is going to be a single procedure, you can get the lumbar MRI done as well. No harm in it.

A Chiropracter would be dealing more so with the superficial muscles and the spasms. I do not think a Chiropracter has more work to do in the pelvic area. Pelvic physiotherapy is a better option here.

I do not think you will have the problem with urination while on pelvic physiotherapy now. Do not be worried about that now.

Please go for the MRI, followed by the pelvic physiotherapy.

Well, another development!!!

My gyno called me to tell me a urine sample I left last Friday cam back with a small amount of bacteria in it, 10,000 - 20,000 cfu/ml Entercoccus Faecalis. The amount of bacteria in my urine wasnt the 100,000+ cfu/ml that she said is usually considered a full blown UTI, it was between 10,000 - 20,000 cfu/ml so they said it is mild low level UTI but I feel like I have to pee every half hour, even after I pee I feel a few minutes later like I have to pee sometimes so even though the count is low, you can still go on an antibiotic right? Ampicillin came back as an "S" meaning the bacteria will be "Suseptable" to it with less than 2 and Penicillin as an "S" and positive 2, so I think I will try the Ampicillin. Only other two that came back with "S" as well were Cipro less than 1 and Nitrofurontoin less than 32 but I have had bad experiences on both Cipro and Nitro (Macrobid) in the past so I think I will try the Ampicillin. It says usually you take 500 mg 3 - 4 times a day for 7-14 days. Any thoughts on how much to take and how long to stay on to be sure to kick it? 7 days, 10 days, 14 days?

I have to go see the doctor tomorrow to discuss which drug I will go on.

I think I got the UTI from taking a hot bath and not showering before. I showered after.
I hadnt showered since two days before and I pee'd in the bath when I was laying in it. Dont know if that let some bacteria maybe from hot bath tub water get into my urethra.
I layed in the hot bath to relax for 20 minutes then showered after but dont think I pee'd again for a few hours.
I also remember laying on the couch later that night and having to pee but holding it for a while cause I was tired and the pain in my side was bothering me so I didnt feel like getting up tp pee. Probably held for an hour or so longer than I should have.
Next morning, boom, constant pee feeling:(
My nitrites and leukocytes came back negative as well but the pee feeling I have had for days and it isnt subsiding.

Question #2. I have been taking D'mannonse as well, 1000 mg every 3-4 hours for the past 2 days but i read on internet it is only good for ecoli bacteria. I have Entercoccus Faecalis. Do you know if D'mannonse could help with that one as well?

Question #3. Lastly, could this possibly clear up without antibiotics or probably not and I should go on them?

Again thank you in advance for any help you can give me with this topic. This is my first UTI...ever and hopefully will go away as quickly as it came!!!

Do not use antibiotics for this

Hello XXXXXXX

You have a count of 10,000 - 20,000 cfu/ml Entercoccus Faecalis in the urine sample test. This definitely does not necessitate the use of antibiotics. In my view, it is not a good idea for you to take Ampicillin. Just drinking plenty of Cranberry juice, along with high intake of other fluids orally will take care of your symptoms. I would definitely not advise you to take antibiotics for this.

The usual course of Ampicillin would be for 7 days. Do not withhold the urge to pass urine. It is not good.

Answer to Question #2 D mannose is said to be good for preventing UTI or urinary tract infection. Yes, it is good for E coli and not E Faecalis. Also, at this time there is insufficient evidence on its use in UTI. Also, the dosage of 1000mg every 3 to 4 hours is high. I would recommend that you DO NOT consume it anymore.

Answer to Question #3 This would more definitely clear up without antibiotics. I do not think you should go on them.

P.S. Enterococcus Faecalis is usually seen in faeces or stool samples. In my view, it is most likely to be cross contamination of your urine sample from the faeces or other errors in processing in the lab.

Yes, this UTI should go away as quickly as it came.
