Can High TSH Level And Headache Be Permanently Cured While Having Pituitary Adenoma?

Question: My wife (39/F) is suffering from high TSH level and pituitary adenoma for last 4 years. She was taking Cabergolin but since pregnancy in November 2016 and delivery of our child in July, 2017, she had to stop Caberlin. Now she again complained head ache and subsequently we checked her prolactin, which is 78. Please suggest a permanent cure.

I can understand your concern regarding to your wifes health.
First we should know when did she stop breastfeeding? As prolactin can be high 2 years after breastfeeding (of course if the normal range is up to 25).
Secondly, she should correct TSH to be sure that high prolactin is not due to high TSH. Be sure that levothyroxine dose is the right one.
Third, she should perform a second MRI of pituitary gland after giving birth to be sure that adenoma is not increased.
If adenoma is still present or increased, cabergoline should be still taken for a couple of years.

1) She is already taking Eltroxin-75 daily empty stomach. 2) Our child (10 month old) didn't get adequate mother's milk since his birth, but still he comes to his mother for breastfeed (whatever he gets) few times daily - Is breastfeeding safe for my son at this enhanced prolactin level (78) of his mother ? 3) Will Caberlin be safe for breastfeeding moms ? 4) Is there any medicine to get temporary relief from headache which is also 'safe' for both mother and child ?

Hi again,
First i would recommend to check TSH level to be sure that the dose of levothyroxine is right.
Secondly, if she has headache an MRI should be performed as soon as possible to be sure that adenoma is not increased as prolactin level can not be judged as she is breast feeding.
Cabergoline is recommended after the first trimester of pregnancy and during breast feeding, meanwhile paracetamol is recommended to be taken for headache during breastfeeding.
