Can Be Soft Tissue Injury Cause Pectoral Tendon Avulsion On My Right Chest From Heavy Bench Press

Question: 47 yo male critical care nurse that is 4 weeks post op from a complete pectoral tendon avulsion (sternal head) on my right chest from heavy bench press. Of note: After the mri was completed and I was diagnosed with the complete tear, I was getting up from bed, not thinking I used my injured side to push off. I felt another pop with less intensity but similar to the original injury. 2 weeks later my surgeon repaired with 3 Athrex pec buttons with fiber tape were used for repair. He said the mobilized just one tendon and didn’t see any other tears. Recovery has been uneventful except now that the sling is off and I adduct my arm about 30 degrees I notice a divot above my axillary fold and the top of my bicep. Additionally there is a very minor new bruise in the deformity area. When I adduct the arm I can also see a palpable pectoral facia cord as well. Upon palpating the muscle in the axillary fold feels just slightly thinner than the normal arm. Now when I flex the pec it does feel like the whole pectoral is activated. There is a small area at the lower lateral border that I can’t tell if it’s not firing or just fat tissue. I have felt no tearing or popping sensations or increased pain or other symptoms. Is this deformity and bruise indicative of a partial failure or tear in the musculotendonous joint. What are your thoughts? Do I need an mri and if so chest and shoulder without contrast or something else? I don’t think my surgeon has much experience with pectoral tears and maybe finding someone more experienced with this injury

Hello,
The bruise in the area is suggestive of a minor soft tissue injury that will heal with time. The deformity that you see can be related to post operative scarring.
It is best to let the area heal properly and plan for an MRI scan in case the bruise increases or there is difficulty in any physiotherapy exercises or at least 12 weeks after the surgery so as to allow the area to heal.
Such changes can reduce as gradually the range of movement increases.

Last question, to be clear. After review of the photos you think that this represents scar tissue and not a minor tear? No mri is needed or should I get one to be sure?

This seems to be a scar tissue and is not like a tear. In case there is any restriction of movement, then MRI scan may be needed. Right now you can relax.
