Can SVR 12 Give Accurate Results About Hepatitis C Infection?

Question: How reliable is svr12 of a hep c cure? I'm svr12 and my hepatologist says I am cured. I am the hardest to treat , geno 3, cirrhotic, high viral load, diabetic. Recent studies say svr12 is pretty much 100 percent accurate as svr24 (the old standard) in predicting a cure. Being the hardest to treat what are my chances that this is a cure? Can I figure 100 percent also?

Cure rates cannot be 100% in presence of Cirrhosis.

Hi Sir,

I understand your concern.

SVR 12 has very good predictability of the cure rates even in presence of high viral load,genotype 3 and diabetic state.

But in the presence of cirrhosis,SVR 12 is not a good predictor.

Survival/Cure cannot be 100 % in presence of cirrhotic liver.

This could be a communication error.

Hello Sir,

I understand your concern.

Let me explain the following lines to you.

You have been on Triple drug regimen of Sofusbuvir,Ribavarin and Peg-IF.

You also stated that you attained SVR12 in Dec 2014.This is a Good sign.

This indicates that you have shown good response to the drugs,as there is 2-log fold reduction of viral load.

Patients who develop SVR12 are the candidates for further extension of treatment to 24 weeks ,since only 65% of patients only go onto develop SVR12-which is only a an indication of early virological response.

The duration of treatment in presence of Cirrhosis is 24 months,hence SVR 24( an indication of late virological response) is attained in approx 97% patients(but not 100% here as well),who are considered potentially cured of the infection.

97% itself is an excellent score.

Hence SVR24 is a better predictor of possible cure of infection( with least risk of recurrence of infection) in patients with Cirrhosis.
