It is now over 4 weeks that I have had this severe pain in lower back and in particular in buttocks and down leg to knee ... it started suddenly whilst on holiday in XXXXXXX XXXXXXX where I was prescribed Naprosyn 500 which gave minor relief and they got me a lumbar belt which did help... since returning to Australia I was prescribed Naprosyn 750... after 3 days I went back as the pain was really wearing me down ... then I was prescribed endone and prednisolone which are helping with pain relief but within 4-5 hours the pain returns ... how long should I continue pain relief without knowing what the pain is? ... after what I have read up on back pain I would hazard a guess that it maybe Sciatica Neuralgia...

Hello again.

If the pain is not responding and recurs when the medication wears off then imaging of the spine should be considered as the next step. The back and lower limbs need to be examined. X ray Lumbosacral spine AP and lateral views can be done to look for any vertebral compression. An MRI can also be planned subsequently.

Sciatica pain is commonly related to a herniated disc. Bed rest in addition to Gabapentin, steroids , muscle relaxants, NSAIDs and physiotherapy are the initial mainstay of therapy for sciatic pain. Other invasive methods such as epidural block are resorted to if there is no response to treatment.

I doubt whether naproxen is causing the cough. Similarly, prednisone is used to counter urticarial in many cases. PPIs like esomeprazole or H 2 blockers such as ranitidine can be prescribed additionally with Naprosyn and prednisone. For the itching topical calamine lotion along with anti histamines (H 1 blockers ) like cetirizine would be ideal.
