Hello Doctor
On a scale of ten I can say 6 so far.
I have started taking acotiamide from today. Hope I can get more relief in days to come.
Doctor, can you please advise what does acotiamide do?
Infact I have made a drastic change to my diet by cutting down oily foods and sweet since I have started taking your medication. Though my FBS is in the range of 125 - 140, I hope I can bring it down in the coming days.
By the way doctor, is there any harm in taking statins for long duration. I have been taking Rosuvastatin 10 mg for 10 years now and my doctor has advised me not to stop it since I have a medical history in the family where I lost my brother at the age of 35. Please advise.

Brief Answer:

Detailed Answer:
Hello again my dear patient,
Sweets should be avoided as you are diabetics however sugar free sweets are ok.
Oil you can use extra virgin olive oil.
Dear statins are very safe and i myself is on atorvastatin 20 mg for more then 10 years and i don't feel any specific side effect however a liver function test should be done every year.
Shift from sweets to sugar free food or recipies.
No banana or very sweet fruit rather take apple and guava pears.
Shift from white rice to brown rice.
White bread to multigrain bread.
Take more carbohydrated before 6 pm.
In evening try to take less carbs.
You sugar would be in control soon.
Acotiamide stimulates postprandial gastric motor activity and gastric emptying by inhibiting AChE without binding to dopamine D2 or serotonin 5-HT4 receptors. In easy words it empties stomach fast hence less acid would be secreted. As formation is less reflux would be less.
Clinically very effective.
Minimal duration is 100 mg thrice for 21 days followed by twice for 3 months.
Have patience in 5 days you are 60% relieved.
Drink one glass of ice cold milk in morning and evening. It being highly alkaline would neutralise acid.
Hope to hear from you soon.
Thanks.

Thank you Doctor for your advice again.

Glad to hear there is no side effects on Statins.
Thanks for explaining about acotiamide. Infact, I havent noticed any change after taking it, however, i guess it should give better results in the coming days.
I have cut down drastically from my earlier eating habits and i hope my sugar levels would be better and I wish my GERD issued would be cured 100%.
How long should I take the esomeprazole Doctor? Would there be any side effects in taking esomeprazole and acotiamide? Especially I have started with the high dose of esomeprazole of 40mg x 2. Tomorrow would be my 10th day. I am hoping for a day when I can stop the medicines without feeling any acid reflux problems.
Thank you again Doctor!

Brief Answer:
follow up.

Detailed Answer:
Hello again my dear patient,
See firstly sir expectations should be practical. There is no magical formula.
Minimum 3 months of treatment would be required initially and after that we could start tapering medicines off.
Among medicines some are fast acting while other are slow ones for ex esomeprazole has fast line of action while acotiamide would show long term effects(may take even a month for initial response).
Initially take esomeprazole for 21 days at high dose after that if your symptoms are relieved till 8 on a scale of 10 i would make it 80 in morning 40 in evening for a week and as per the response of body keep on decreasing the doses.
There are definitely side effect of every drug.Even air we breath contains numerous bacterias and allergans but what we consider is ratio of pros and cons.So keep your mind calm these are all very safe medicines.
Don't google too much.
Yes things would take time but i assure you,You would be much better soon.
Regards.

Hello Doctor

Thanks for your advice.

I have completed my 10 days and I feel far better. My continuous cough has completely stopped. Heartburn is nil. Only at times I feel some sort of discomfort inside the abdomen, mostly after taking food. Otherwise I feel really goood.
I dont know whether I have upset you with my queries on side effects of these medicines. Thank you for clarifying that these medicines are safe. As patients, we are always anxious and curious about the curing process as well as the side effects of the medications we take. Hope you bear with me since I have multiple health issues and take several medicines either together or separately. Moreover, we have met with numerous doctors and all of them prescribe several medicines. Unfortunately, our lack of knowledge of the pros and cons of such medicines make us ask doctors such questions which some times upsets them.
Apologies if I have done so.

As I am a Muslim, we will be following a month of fasting from mid of May, which would be around 15 hours a day. I would like to take your advise on how my dosage at noon can be taken, which is digezyme plus the antacid.

Please advise.
