Can HIV Be Contracted Through Unprotected Intercourse?

Question: Last saturday (2 days) Before i have sex (first time) with sex worker, but i was using condom.but in between condom was broke..i didn't see few minutes(appro. 3 min)..now i am afraid of any sexual problem come to me in future like hiv aids..i am totally upset and almost i losy my hope..please advice what should i do

All the sex workers are not HIV positive. Less than 30% could possibly be HIV positive throughout the world as they are also more concerned with their own health they are more conscious to have safe sex.
All those exposed to positive individuals are not having HIV. Nearly 30-40% of spouses of HIV positive individuals remain HIV negative in spite of their regular sex relationship for years together and they are called as discordant couples. Moreover the risk of HIV transmission is 4-5 per 10000 exposures.
Nowadays HIV is not a life threatening disease. It is well manageable disease with drugs and lifestyle modification and the individuals with HIV positive can live longer as their counterpart without HIV.
Breakage of condom can increase the risk to an extent and not like that of unprotected sex.
You will be unlikely to have HIV/STD.

Sorry doctor..i forget to ask one question..is there any need to take HIV test? when it will be taken. I searched in internet about test..it will show the result perfectly after the 3 months period from sex taken..is this true?

It is better to rule out HIV /STD with the help of blood investigations. Otherwise there will be a little tension prevails for ever. We will tend to attribute this incident for any future illnesses.
It is true that the commonly available HIV tests are better reliable only after 12 weeks of the sexual activities. There are some special tests available with certain bigger labs like SRL Ranbaxy or Lal Path to rule out HIV 1 infection within 4 weeks after the exposure. My suggestion is that you wait for 12 weeks and have the 4th generation DUO HIV test which will be reliable to the extent of 99. 8%.

You had tests for HIV, Syphilis and HSV. But all these test reports can not be reliable as you had your sexual exposure just 4 days ago. Each test has its own window period (time taken for the appearance of antibodies in the body against any organism) and within 4 days antibody formation can not be expected. Your test confirmed that you were not having HIV, Syphilis or HSV infections prior to this exposure. Better to repeat the test after 12 weeks.

I lost my patience doctor..so only i go to siddha doctor and he only advised to me to take the test..and also he gave the antibotic tablets for one month and it should continued for minimum four months..my doubt is that whether can i continue the tablets for four months or i can wait another 3 months and after i will do the test what you recommened duo hiv test..if something goes wrong could i continue the treatment.. for taking DUO HIV test, should i need any doctor recommentation letter..kindly advise me doctor

Management with medicine is not possible after 72 hours of the exposure to prevent or abort the possibility of disease in presence of an infection.
You need not postpone your marriage. Your risk for HIV/STD is minimal only. You can take up the following test 4 weeks after that activity.
1 DNA PCR TEST for HIV 1 infection.
2. Qualitative RNA PCR test specific for HIV 2 infection available only with certain labs
3. STD Profile tests
I hope all will become non reactive. Then you can go for your marriage happily without any doubts.
Nowadays HIV is a well manageable disease and people can lead a normal healthier life for 25 years or more with treatment.

Hi doctor,

Today is 24th day from the exposure,

Today i went to hospital and have taken the test..Please review the reports doctor as i attached. result is negative only.Thanks god .and doctor said to me ,No need to take any confirmation test after 3 months ( from the period of exposure). and he suggested two tablets for next 3 months , Namely 1)SHI LI capsules with goji berry extract 2) shandian ( antioxdant softgel capsules)

Should i take again test after 3 months ( from the period of exposure) ?

Sore throat infection for some other reason, not related hiv for me , correct doctor?

Now can i prepare for my marriage doctor.

Welcome back.
The test you had for HIV, is an antibody based test. Antibodies against HIV will appear 6-8 weeks after the exposure in presence of an infection and this may be delayed up to 12 weeks in some cases. This is called as window period. So your test will be more reliable only after 12 weeks. If you want it earlier, you can go for antigen detecting test like DNA PCR, P24 antigen test. Qualitative RNA PCR separately for HIV 1 and 2 as the combination antigen test is not available for both. Those tests also considered more conclusive after 4 weeks even though it can detect HIV earlier.
Your other tests were normal. Even VDRL will be reliable only after 28 days.

Today is 40th day after the exposure

I have taken following hiv test for 33rd day after the exposure,

1.VDRL
2.HBsAg
3.HIV 1 & 2 Elisa
4.HIV RT PCR (Qualitative)

Reports are attached for your review doctor,
Kindly advice me , what is the accuarcy of this test after 33rd day exposure.
Doctor told to me, HIV RT PCR test are conculsive in after 4 weeks of exposure

Your risk for HIV is low. HIV 2 is a very very rare possibility. You can ignore the same.
1. In spite of my assurance, if you want to have test for HIV 2, Qualitative RNA PCR exclusively for HIV 2 is available only with few laboratories. It would be reliable by now.
2. Increase in Uric acid level is very minimal and can be ignored. It has no relation to HIV.
3. If there is no joint pain, you can ignore the rise in Uric acid level. Rheumatlogist or Orthopedician are ideal specialists to approach for joint pain.
