Brief Answer:
HIV can't be judged by symptoms

Detailed Answer:
Thanks for your question. I am Dr Matin and will be answering your queries.

I understand the sex worker was a female one, though you have spell her as 'he'.
First of all, let us understand the fact that HIV can't be judged by symptoms alone and nobody can be labeled as 'HIV +'. So no sex worker can be labeled as HIV +, just because she had eruptions on her leg.
Secondly, if there was no leakage or break of condoms, then there is virtually no chance of catching HIV.
Merely touching semen by hand can't transmit HIV to you unless you have some cuts/sores over your hand.
Transmission of HIV through skin/mucosa depends upon a lot of variables and in your scenario the chances are negligible.

As I wrote above, symptoms have no role in diagnosing HIV, so having sore throat after 11 days of exposure has no significance in the diagnosis of HIV.

The only way to confirm is HIV tests, which may be suggested for you in case you want to buy peace of mind.

Hope it helps.

can I trust in condoms because lots of people speak condoms does not prevent hiv transmission if person have high transmission load or virus that are newly infected or not taken any medicine chance it means can I trust for remaining life
doctor
Answered by Dr. Matin Ahmad Khan (48 minutes later)
Brief Answer:
It can be trusted

Detailed Answer:
Thanks for your response.
Condoms can be trusted, if it is not torn or slipped during the sexual act and the variables you mentioned --high viral load, new infection, prior history of taking anti HIV medicines, do have affect on the transmission of HIV only when the condoms(barrier) have not been used.

Hope this helps.

Brief Answer:
Double condoms are not advised

Detailed Answer:
Thanks for your response.
Using double condoms do not give additional protection.
On the contrary , it can be torn easily due to more friction and it is not even easier to put on extra condom , over another.
Hope this helps.

sir,
I told my incident our last discussion!!!
because of may anxiety and few symptoms I decide to go for test, where laboratory asked for DOCTOR which test is suitable at 19 day from exposure??? they told me,
DNA PCR test is best is your situation and test is done. After that result is come back NEGETIVE but I don't no they tested for both HIV1 AND HIV2.
sir I have still face the anxiety because of from yesterday i have mouth ulcers in my mouth ,is one of the HIV1 or HIV2 that lab missed to test???
sir please help me is DNA PCR is conclusive at 19 day from exposure
and also here that the HIV2 is very slow replication is because of that HIV2 not found during test???
but laboratory said u no need of further testing..
doctor
Answered by Dr. Matin Ahmad Khan (23 minutes later)
Brief Answer:
Not conclusive

Detailed Answer:
Thanks for your question.
There is a protocol for an HIV test -first at base line , and for conclusiveness after 90 days of exposure.
So technically DNA PCR is not conclusive @19 days ,and you will need another test as per protocol @90 days of exposure to be absolutely sure of your status.
DNA PCR is done for both HIV1 and HIV2 , no worries over there.
You are becoming over anxious there but it is to be reminded again that HIV can't be judged or diagnosed by symptoms (be it rashes/mouth ulcers in your case) and needs to be confirmed by an HIV test .
Hope this helps.

sir,
so I still have chance of HIV infection
how many % DNA PCR test is reliable and sensitive at 19 days from exposure in healthy person without symptoms
doctor
Answered by Dr. Matin Ahmad Khan (51 minutes later)
Brief Answer:
HIV infection should not rely solely upon the result of an HIV DNA PCR .

Detailed Answer:
Thanks for your response.

PCR sensitivity(power of picking up positive cases) varies and it could be up to 96%.
Sensitivity @19 days has not been studied, so can't comment upon that.
My only submission is that diagnosis of HIV infection should not rely solely upon the result of an HIV DNA PCR.
A positive result should be considered in conjunction with clinical presentation and additional established diagnostic tests prior to establishing a diagnosis.
A negative result indicates only the absence of HIV DNA in the specimen tested and does not exclude the diagnosis of disease.
So, it means that whether positive or negative DNA PCR, one has to confirm the result as protocol, finally at 90 days of exposure.
But since one DNR PCR is already negative, so by experience I can assure you that the result of your 90 days test will too come negative.

Hope it helps.

Brief Answer:
Symptoms have no role

Detailed Answer:
Thanks for your kind response. Here are your answers.
1. Symptoms have no role unless tested positive.
2. It varies from person to person, can't really predict.
3. Timeline of symptoms has no significance.
4. Symptoms without positivity no role - can't judge really.
5. Again same answer -no meaning
6. A negative test which is done (4th generation which tests antigen and antibody both) these days @ 90 days of exposure is conclusive of negativity.

I guess there are no more questions and I wind up here.
Hope this helps.
