Question: Hello doctor,
I m a 27 years male from Tamil nadu and ten days back I had a sexual exposure with a csw girl in malaysia she is from cambodia. I didn't have vaginal and anal sex and I had protected oral sex with her with latex condoms. two days before the exposure I shaved my penis and had two small nicks in scrotum and it bleeded and stopped with in seconds and there is no visible cut or scratch. before performing oral sex the girl showered me and then she showered and after that she rubbed her body with my body and during rubbing I don't know whether her vagina touched my penis. I experienced a small wetness near my thighs and I don't know whether that's her vaginal fluid. I m very much worried after this incident.while performing oral sex she licked my scrotum several times. after one day from exposure I m feeling very thirsty and my tongue is dry often.after seven days from my exposure I am having red rashes inside penis and thighs.are these symptoms of hiv? if during body rubbing her vagina touched my penis will hiv pass through that? if that wetness us vaginal fluids and if it touches penis is that a risk? she licked my scrotum and will hiv pass through that scrotum which had invisible nicks? pls help doctor

Hi.

There is no risk of Hiv through this mode. As far as cuts on scrotal skin are concerned there is no risk through them too. The virus cannot enter through minor cuts which are more than a few hours old and which are not actively bleeding.
The rash on inner thighs is most likely tinea.

The rashes are most likely fungal in origin and not due to an STD.

I suggest you to use Canesten-HC cream twice daily for 2weeks for the rashes.

Hi.

Genital herpes usually presents with discrete well defined erosions/ ulcers in the genital area. The sores are painful and also have a burning sensation.
Without any visible sores it is difficult to think of a diagnosis of herpes. Just burning sensation does not suggest a diagnosis of genital herpes.
You may upload an Image of the rash that you have in the genital/ groinl area, so that I can confirm whether it is herpes or not?Or is it something else?

Thanks doctor. I am very much worried. From yesterday the rashes are increasing in numbers and now there are more than ten rashes in my body. Two in hands, two rashes near stomache, one in thighs, three rashes near chest area. Yesterday I went to a dermatologist and he enquired about me and sexual activities and he suspect the rashes to be scabies. He advised me to do a sypillis test and it came back as negative. And he advised Me to do a hiv elisa test due to my anxiety which came as 0.3 and neg. Is 0.3 in ten days from exposure a high value? Will it increase day by day? Pls advice me doctor. Now afternoon I noticed some rash like thing inside my mouth. I have attached the image of those. What's the reason for these rashes. I am feeling very hot in thighs for the last two days and it's creating a burning sensation in thighs for some time. Are these symptoms of hiv? Or herpes ? Kindly clarify doctor. pls help me doctor.

I have reviewed the Images
The rash on glans is most likely a candidal/ yeast infection. I suggest you to use candid- B cream, twice daily for a week.
Use the same cream for thighs as well.
You Hiv Elisa is normal but it is too soon to test for it. Hiv Elisa should be performed at least a month after an exposure. However, since your exposure does not entail any risk of Hiv I wont even suggest testing for Hiv.
Certainly the symptoms and images don't seem to suggest genital herpes, either.
The red spot on tongue is most likely due to tooth bite and it would resolve.

I have these rashes all over my body it is inside the penis and it is spread in thighs, hands and chest. the rashes are not itchy and it is painless.i have read in internet that If the rashes are yeast or fungal infection the rashes will be itchy. but my rashes are not itchy if the rashes are not itchy then it is not fungal or yeast infection? pls clarify doctor. will the fungal or yeast rashes spread all over the body as exposed in me ? pls clarify doctor. I took a blood test two days back and the wbc is 12.7 and the doctor said it is slightly high and it may be due to body responding to some infection. will the wbc count increases when body is fighting yeast or fungal infection ? pls clarify doctor.

Doctor i have attached the images of my rashes. it is in chest and shoulders and the rashes are not itching and painless. they are increasing in no's and now i m having 15 rashes in my body. i m very much stressed and worried pls clarify me the causes of the rashes and its type. are these symptoms of hiv or any other std's? pls clarify waiting for ur reply with anxiety.

The morphology and distribution of the rashes suggest that it is likely scabies. It is certainlt not an STD.
Itching is a subjective symptom and some patients may not itch very much.
I suggest you to take empirical treatment for scabies i.e single whole body application of permethrin 5% cream which should be kept overnight.

Hi.

Your wbc count is slightly increased but it is not very deranged. Normal wbc count has a range from 4000-11000 cells/ cubic mm. This slight increase can happen in instances of minor infections like e.g a skin infection or dental infection. Scabies has an incubation period therefore it can manifest after a few days of infection. After scabicidal treatment with single application of permethrin 5% cream i suggest you to use calamine lotion for a few days over the rashes.

Thanks doctor I am using the permethrin cream as prescribed by you. I took a hiv rna pcr real time test on 12th day from exposure and the results came back as target not detected. the test methodology is Abbott real time pcr rna. how far are my test result reliable ? can I get hopes that I am not infected with hiv from my exposure? pls advice me doctor

Pcr for hiv rna is a very reliable test beyond quite early after infection i.e majority of those infected will have a positive result at about 2 weeks (range 9-13 days) from sexual contact and therefore a negative test result for hiv pcr rna at 12 days can be considered fairly reliable.
