Question: Hi Dr., I am not able to sleep properly since last few weeks.Let me explain you the sin i committed recently.
Exactly five weeks ago, I had an one night stand with a call girl. I am going to explain everything in detail so that I will get an accurate answer from you. She looks very clean and healthy. I wore two condoms (mood condoms), we tried to do vaginal sex. She tried to put it inside, there was no penetrative sex from my side. It continued for a 1 or 2 minutes but I stopped it suddenly as I was little scared. After that I did a lot of deep fingering with my bare fingers (my fingers have no cuts, i don't see any cuts around her vagina as i mentioned she also looks clean). Then I kissed her whole body (no vaginal, no anal, no french kiss). At some point I kissed her lips also, just the outer part of lips, I made sure that the tongues were not touched. Whole night we hugged each other and slept (I wore a short). Out of my curiosity, the very next day I convinced her and took her to lab for hiv test. And to my bad luck, the result came out to be positive (attached is the file, and we directly opted for western blot test, no other test performed on her). Kindly evaluate my risk after this exposure, I am really scared to death now.

Protected intercourse has almost nil risk for HIV and STD. You wore two condoms. Penetration not took place. Then there is no risk at all.
Other than blood transfusion, needle prick with an infected blood, Anal, vaginal and oral sex, all other activities (including kissing, hugging and fingering) carry a negligible risk as per CDC XXXXXXX
Regarding HIV test Western blot is considered more reliable and confirmatory than ELISA or rapid test as it detects multiple antibodies against HIV.
Every HIV positive couple, whether the spouse are HIV positive or not, are allowed to have protected sex. Their non infected spouse remained HIV free in spite of their regular protected intercourse.
So your risk is negligible only. Nothing much to worry.

Thanks Dr, May be I am little worried because of the fact that I did all these with HIV positive person. I am cursing myself since last few weeks. After this incident I have been reading the HIV symptoms in early stage over the internet. After few days of this incident, I got pain in my hands which I usually get sometimes (I used go to gym and stopped few moths back, I took tablets and it's gone immediately). After few days, may be in the week 2/3 after exposure, I had a headache lasted for few days (I don't know the reason, actually I am spending 4/5 hours every day over the internet reading all these, and always thinking about this mistake I did ). I would like to check with you, by any chance (in the worst scenario), do these all under ARS symptoms? It's been 5 weeks now after that one night stand, apart from the above two mentioned symptoms, I haven't had any symptoms. Can i relax now? I read somewhere that in few people it may take up to 8 weeks to develop ARS symptoms? Could you please throw some light on this.

I understand your concern.
First of all, stop looking for materials from net. It will really confuse you as anything can occur in HIV. At the sametime vague symptoms are not anyway specific for HIV and they can occur to anybody without HIV. The blood test for HIV can rule out HIV.
Your symptoms are unlikely to be due to HIV. Your chances of getting HIV is almost nil. Then why should you worry about HIV possibility?

DNA PCR for HIV1,RNA PCR specific for HIV2 infection rule out HIV 1 and 2

You can take up either qualitative RNA PCR or DNA PCR to rule out HIV 1 infection and ask for a seperate qualitative RNA PCR specific for HIV 2 infection availabe only in selective centers (SRL Ranbaxy, LAL Path) to rule out HIV 2 infection. If both the tests come non reactive, it is certain that you are free from HIV 1 and 2 infection. These tests become conclusive by 4 weeks onwards. So you can take up these tests by now confidently and they will be very much conclusive. I am sure both will come non reactive for you.

Hi Dr,Thank you so much.

As we can see it in the report the girl is detected as HIV1 positive. So can i opt for only DNA PCR to rule out HIV1 (Or) opt or both RNA/DNA PCR?

And also one more thin I would like to ask is that I heard a false positives about DNA PCR. I really don't want to take any chance on this. Do you recommend any other test along with above mentioned tests such as HIV DUO and P24 Antigen tests?

Sorry for troubling you y asking many questions, but please understand my situation. I am not able to concentrate on any other thing.

As you rightly pointed out when the girl is HIV 1 positive, it is enough for you to rule out HIV 1 alone through either with DNA PCR or qualitative RNA PCR for HIV 1 infection.
False positive with DNA PCR is rare and less than 1 %. No test is 100% relaible including 4th generation HIV DUO test by 6 weeks.
At the same time almost all the tests are highly reliable up to the extent of more than 99%. Less than 1% false positive is almost negligible and can be ignored and I had yet to meet one such instance in my practice of 30 years.

DNA PCR would cost around INR 2000-2500. 4th generation test will be around INR 1000-1500.
You can contact any big laboratories at XXXXXXX They will send the specimen to higher laboratories like SRL Ranbaxy, Lal PATH, PATH CARE, VIMTA LAB and you can get the result within 2-3 days. If you know the collection centers of the above said Higher labs, then it will be little cheaper for you.

Thank you so much doctor. I am little relaxed as you said chances of getting HIV is almost NIL in one of the above discussions. One last question I would like to ask you is that what percentage of people especially man would exhibit ARS symptoms with in 6 weeks after possible exposure. I know the fact we can not judge HIV by looking at the symptoms. Just to ease my mind since I do not develop man HIV symptoms like Flu like fever, sore throat, rashes,diarrhea etc..

First of all, it is no longer called as ARS. It is Acute infection or Primary HIV infection or Sero-conversion illness. Symptoms of acute infection will be present with 50-70% of the infected individuals and not with all who got infected. But only the blood test result is final. Without a positive blood test no body can fix the diagnosis of a HIV infection .

Hi Doctor, Thank you so much for your detailed answers to the above thread. I have tested for both the tests, DNA PCR and HIV DUO (P24 and ANTIBODIES) after the 6 weeks, both came out to be negative. I am really relaxed now. But I read that only test after 13 weeks is conclusive. Do i have to test again after 13 weeks?

HIV 1 infection has been ruled out beyond any doubt. HIV 1 infection is common throughout the world except in certain parts of XXXXXXX up to more than 99%. HIV 2 incidence is less than 0.5%.

Your DNA PCR and P24 antigen part of the Combo test had ruled out HIV 1 infection beyond any doubt. If you want to rule out HIV 2 infection also (remote possibility) you have to take up HIV 2 RNA PCR (available only in few places) or you have to go for any antibody detecting test or another combo test 12 weeks (window period) after the exposure.

AS that girl is positive only for HIV 1 infection there is no need for HIV 2 infection. You need no further test relax. As you tried for protected sex, you need not worry at all. Fingering has a negligible risk. Anyhow test ruled out this negligible possibility also.
