Question: Hi doctor,

I went to prostitute five months back. After that i feel really worried about HIV infection.
Let me explain about that. One prostitute hold my penies and put condom with using her hand.
i made me fully nude and herself also nude. Then i tried to insert my cock with condom. But in second of time i removed my penies and asked her to remove condom and do handjob with her hand. She gave me handjob without condom and i ejaculated . Then another one prostitute girl game and put condem in my penies than i tried to insert but my penies totaly small at that time due to handjot ejaculation by prostiute girl one. So second girl also did hand job ejaculated. Now my doubt is handjob without condem with her hand is any chance of Infection. Because that vagina fluied may still in her hand due to touching her vagina by herself or removeing my conden with her hand. also i kissed her boobs on that time. In my penies no wound are injury on that time. After 3 months i got fever and stomoch upset and now after 5 months i noticed small black patch on penies inside skin but no pain or no lump now(am scared about Koposi Sarcoma). But am not sure it is happened recent days or it was been already. Also i feel some pumping feel in centure of my chest while am walk. No breathing problem. Pls suggest . Do i need to take hiv test. i cant consentrate on may daily activities like work family anything due to this.

Brief Answer:
No need to go for HIV test as the risk of getting an infection is very low.

Detailed Answer:
Dear XXXX,

Welcome to HCM.

HIV is transmitted through the mucosal linings in the mouth, vagina, penis or anus come in direct contact with HIV infected blood or bodily fluids like semen, vaginal fluid, saliva or any other.

In your case, there is no cut or abrasion or scratch on your genital skin then you need not to worry much about HIV. Also, the survival of HIV virus outside the human body is very difficult and it will die within hours only. HIV can not get transmitted into blood stream through bare hands.

HIV can not get transmitted by body touching or kissing. As far as fever, stomach upset and black patch on penis inside the skin is concerned, it might be due to some other infection for which you can consult your local physician and take some symptomatic treatment or go for any investigation if required.

As the risk of getting an infection with HIV is very remote or negligible, I don't think you have to go for HIV test. Be relaxed and move ahead in life.
