Can Benazepril And DIltiazem Cause An Atrioventricular Block?

default
Posted on Mon, 2 Jun 2014
Question: I am 64 yrs old. Female.I have diabetes type II. I just had and Electrocardiogram and Echocardiogram that showed up abnormal.
I take Metformin ER 500 x2 a day, Benazepril 20 mg x1 daily, Diltiazem CD recently raised to 120/24 x1 daily. Just recently given Pravastatin 20 mg. x 1 daily.
Electrocardiogram showed sinus rhythm,QRS widening,AV Nodol heart block, 1st degree AV block and poor R wave progression. I have been reading on my meds and I noticed that they can have something to do with me taking Benazepril and DIltiazem with an AV Block. SHould I be taking these med? Both meds say not to be used with AV block. I was on Metoprolol and Hydrochlorthiazide for a long time. Could this have caused AV block?
Echo showed trace AV regurgitation sclerosis, diastolic dysfunction, abnormal for age,Atrial enlargement left 4.5 I am a little worried now that I don't have a Primary with enough knowledge on the meds he gives me with my disease. Are these just worsening my heart health?XXXX

Brief Answer:
Diltiazem could have caused block

Detailed Answer:
Hi
Welcome to Healthcare-Magic
Greetings of the day

Dear Ms

First and foremost let me reassure there is no cause for concern.
Among all the anti-hypertensive that you have been using , it's Diltiazem which could be the culprit. Diltiazem being a calcium channel blocker may affect the conductivity of AV node and in some individual may cause or facilitate AV block.
It's very difficult to say whether the AV block has been caused by Diltiazem or due to co-existing heart condition. The block has been more likely to have been caused by co-existing heart condition. Atrial enlargement may predispose to AV block.

Diltiazem may still be continued in 1st degree AV block without any ill effect on heart. Being in sinus rythym it's unlikely to cause any problems. All that you require at present is good follow up and regular assessment of blood pressure at least twice a week, quarterly assessment by 2D Echocardiography and ECG with long lead II.
Rest be assured there is no cause for concern.
In case you require any further assistance, will be glad to assist you.

Take care
