Brief Answer:
Important to know the diagnosis.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/61 - beginning of a cyst right outside of anus - wants to know whether Azithromycin will help - not visited a Doctor nor taken any medicines.
Since you have not mentioned I take that you do not have any symptoms.
Please let me know a few more details for me to assist you better:
Since when is the cyst present?
Any symptoms of pain, tenderness, bleeding?
is it hard or soft and compressible?
Can it be reduced inside anus?
Any other information which you may please share.
If possible post clear and focused photos as it sometimes help for on the spot diagnosis.

If there is a swelling that is soft and supple that can be pressed without any pain, this is most probably an external pile/'hemorrhoid. No antibiotic is needed.
If this is a small swelling that is fir, painful or tender, you may need an antibiotic but Azithromycin is not the drug of choice in such cases.

It is most important to know the exact cause of the swelling to have proper management as this can be a small abscess, boil, cyst as you have mentioned or a beginning of an abscess.
Per-rectal examination and proctoscopy preferably by a General Surgeon give the best chance to have a proper diagnosis.

I hope this answer helps you.

Thank you for the response. There is very mild discomfort, it is hard, noticed yesterday. Have had in past, 20 years ago. Would like to avoid going to a doctor, I went to General Surgeon last time I had but with my insurance there are so many hoops to jump through to get there. The time before that I went to a family practice doctor who made the experience abusive. Thanks in advance.

Brief Answer:
Please take Azithromycin and an anti-inflammatory

Detailed Answer:
I can understand the hardship you might have gone through, albeit 20 years ago.
You may please take Azithromycin if it suits you.
Also take an anti-inflammatory like Ibuprofen if it suits you.
Do not fondle or try to pop; it may spread the infection.
Take care.
If the swelling increases in size or discomfort, please visit your Doctor.
