Brief Answer:
dear sir

Detailed Answer:
am sorry for your patient, but i can see that there is an infection in the brain that leaded to coma. this source of infection is treated in a very good manner with excellent coverage of antibiotic.
the treatment given is more than perfect
the main problem is to find this source of infection, all what was done is doing CT brain, MRI brain and CT of the abdomen.
I can add three things :
MRI chest, abdomen and pelvis
cerebrospinal fluid aspiration
bone scan
this three tests must be done as early as possible

i hope you find my answer quite helpful...

Thank you doctor for the answer. It is beileved that the infection took place during a blood transfusion as the patient suffers from Thalasemia Major Beta and he needs a blood transfusion every 3 weeks.

Can you please explain further regarding the followings:
1- Types of antibiotics needed?
2- Amount?
3- Duration?
4- Any other important information related to the treatment.

Thank you,
doctor

Brief Answer:
dear sir

Detailed Answer:
AS i said before, the antibiotic your patient is taking is more than enough, as he is taking 2 kinds of antibacterial ( vancomycin , meropenam ) and one antiviral ( acyclovir), so i advise to continue on the same kind of medication..the usual doses are:
meropenem 1 gm twice per day
vancomycin 500 three times per day
acyclovir two times per day

this coarse must be for at least days

i hope your patient will be OK soon....

Brief Answer:
good mornind sir,

Detailed Answer:
am sorry for not being clear for you, this antibiotic coarse will at least 10 to 14 days. The reports of the patient you sent does not indicate where the source of infection come from. That's why I sent you more investigations to do.
I personally prefer to do blood culture, but in your patient's case it will be useless as he already started on antimicrobial regimen.
