Are Panic Attacks, Drowsiness And Lack Of Alertness Signs Of Brain Tumor?

Question: Hello doctor
I was suffering from major anxiety disorder and obsessive compulsive disorder for 3 years
I was having obsessions of having HIV and hepatitis which got over on its own
Now I was ok for the last 2 years but was just having fear of flying in aeroplane
Last month I took a long flight and hot too much anxiety
Since then I got three panic attacks and went to emergency and they gave me lonazep
Now for the last two days I m just feeling lack of alertness and drowsy
I feel as if I m drunk all the time
I don't know what's wrong and getting more anxiety because of that
Doctor prescribed me pretex and Etaam for 7 days
I am failing to understand is it my anxiety or actually something is wrong in my brain ?
I mean some Tumor or any other problem
I also took lot of medicine in the last two weeks as I was suffering from cold

Thanks for writing to us.

You are likely suffering from anxiety disorder and mostly it is panic disorder. our doctor has prescribed pretex ( I cannot get it what it is, it is likely paroxetine, please clarify with the dose) and etilam (etizolam- please mention dose of the same). These are good medicines for anxiety disorder. You have also taken lonazep (it is clonazepam - please mention dose and how much time have you taken in last week).
Apart from these you have also taken medicines for cold and those medicines likely include anti histaminic. Please provide name and dose of those medicines also.

All above medicines can cause sedation and dizziness as side effects and taking all together can cause sedation which is troublesome and that is why you are feeling drowsy.

No possibility of tumour is there as none of the mentioned symptoms support this. If you any symptom like have loss of consciousness, ear, nose bleeding, diplopia, severe unbearable headache, convulsion; then we can go for neurological evaluation otherwise it is not at all needed.

Please revert back to me with details of medicines as asked like dose, frequency, generic name.
If you have undergone any investigations , please attache the reports.

I have been taking emanzen d ,Allegra and mucinac along with am dilute cough syrup for 14 days starting 8th XXXXXXX
I got panic attack (that's what I think ) 3 times
First I started feeling drowsy and shortness of breadth then I think my anxiety increased a lot so I went to emergency and they gave me lorazep 0.25 mg
Day before yesterday I went to a physactrst and he gave me etilaam 0.5 mg and another medicine 12.5 mg I guess pretex (Ssri)
Also j am suffering from lots and lots of gas I hope that's not the reason for drowsiness or lack of alertness and anxiety there after

Thanks for information.

Cough syrup can cause sedation. However that has been stopped.

Etilaam, paroxetine and lonazep all can cause sedation. At present take etizolam in half dose and peroxetine at night. If you have panic attack then only take lonazep.

Inform your doctor about your drowsiness and lack of alertness.

Anxiety in general can cause more alertness and restlessness. But few patients due to low mood and lack of sleep patient can feel drowsy.

Your doctor can do neurological examination and reassure about not having any neurological problem.

What could be the neurological problem ?
Can lack of alertness be because of anxiety or I am getting anxiety because of lack of alertness ?
After taking lonazep I feel fully alert ?
Can lack of alertness be sign of any tumor or something ? (No other symptom )
Is etilaam and lorazep same ?
Doctor told me to take
Etlaam 0.5 mg and pretax 12.5 mg ?
Should I make it half or it is ok ?

Also I had no problem until three week ago
but I took a long flight and got too much anxiety in the flight
Could it be the trigger of this anxiety disorder

Neurological problem is not there. I mentioned only to rule out your concern and reassure you.

Lack of alertness due to anxiety is in your case.

No lonazep will help to make you relax after panic attack.


Only lack of alertness cannot be sign of any tumor.

Etizolam and lonazep are both anti anxiety drugs but lonazep can cause more sedation and more long acting than etizolam.

Make only etilam 0.5 mg half. Do not make pretax 12.5 half , just take it at night.

Yes long flight is precipitating factor of your anxiety disorder. You have past history of the same. You had recovered in past so again you will be recovered soon.

I m taking Etilaam 0.5 mg and Pexep 12.5 at night and today evening I went for a walk and was talking to a friend of mine
I started feeling shortness of breadth , lack of consciousness like fainting and felt either i have low sugar or I m dying of brain Timor or heart attack
I ate something and had etilaam and became normal in 20 minutes
Is it sure sign of panic attack?
Can I prevent panic attack without medicine?
I recovered from obsessive compulsive disorder by Exposure and response therapy
What can I go to get over panic attacks ?

Good morning.

The episode you have mentioned is panic attack only. It resolved after 20 minutes with etilaam and that confirms the same.

Yes psychotherapy also can treat panic disorder. Panic attack can be relieved with medicine like etilam or deep breathing exercise. Do relaxation exercise. During panic attack take deep slow breathing. Inhale for 3 seconds, keep breath in for 3 seconds and exhale for 3 seconds. Do it for 10-15 minutes. You will feel relax.

Simultaneously also go for CBT- Cognitive behavior therapy. CBT is specifically used to correct automatic thoughts and distorted cognition. In CBT psychiatrist/psychologist helps person to identify his/her automatic thoughts and faulty behavior which would ultimately be modified by person with active efforts and help of therapist. It also includes time management, daily sleep, meals and exercise timing.

You can ask your doctor for CBT.

Thanks for follow up.

I am using etilaam in my patients very frequently in all anxiety spectrum disorders. Usually after symptomatic improvement I tapered it gradually and stop in 2-3 months. Though it is a benzodiazepine it is no or very less addictive. Person can stop it easily without significant withdrawals. You can take it for few days under your doctor's guidance.

Frequent episodes of panic attacks is called panic disorder. My choice of treatment is same paroxetine, etilaam with CBT. Yes many patients are cured with this treatment. You had come out of OCD so you are also likely cured from panic disorder.

I am feeling pressure in head and eats all the time and that is causing me anxiety and panic attacks ?
Can this pressure in ear and head because of sinus ?
I hope I m not taking this medicine for no reason and my treatment need taking care of ear and head pressure ?
Can I stop medication for some time

Thanks for follow up.

Pressure head can be due to sinusitis but frequent panic attacks cannot be due to the same. In that case we need to rule out sinusitis. You need to consult ENT specialist for clinical examination and if needed X-ray PNS (para nasal sinuses) can be done. By chance if it is there, we can treat it fast.

I do not recommend to stop medicines without consulting your doctor. Better to rule out sinusitis first.

Yes ultracet can be taken with etilaam. However it is pain killer tramadol and prescription drug. You can use it for severe pain.

What I feel is since I m feeling pressure in head even after taking etilaam but only my anxiety stops
I feel since I m hypochondriac maybe when I felt pressure in head I got anxiety and then panic attck anyways I will go to ent tomorrow also I was suffering from bad cough and gold when these panic attacks started
Also I hope etilaam is not addictive and if I stop I won't have withdrawal syndrome

Hi,

Yes you are right if you have headache and you feel anxiety about that you may have panic attack. In that case treating basic cause of headache is needed to cure the problem. Let me know after consulting doctor for sinusitis or other ENT problem.

Etilaam 0.5 mg is very low dose and you can easily stop without withdrawals. If you will take it more than 4 weeks you can reduce to 0.25 mg for one week and then stop without any problem.
