I am Dr XXXXXXX ( dermatologist) and I hope I can understand your concern.

First of all ,I assume your diagnosis of scabies is confirmed by your doctor and I will guide only about management part.

Scabeis is a parasitic infection caused by mite.

It is a highly contagious disease which spreads rapidly among family members and close contacts .Disease is associated with severe itching particularly at night.

Mainstay of treatment is permethrin cream .It has to be applied below neck and a single application is advised to whole body.

Supportive treatment includes a permethrin containing soap and antihistamincis such as levocetrizine and hydroxizine. All family members need to be treated simultaneously. You may require a prescription for this.

There is no requirement of injection for scabies since most of patients will recover with above treatment.

Levocetrizine is available on counter.

It is available as tablet xyzal .It should be taken in morning .

Hydroxizine tablet which is commonly marketed as atarax is available on prescription .It has two strength 10 mg and 25 mg ,you can start with 10 mg tablet at night and if itching is not relieved you can increase it to 25 mg.

If it is impossible for you to consult a doctor I will advise you only to take xyzal tablet two times daily.

You can use any antiseptic soap instaed of permithrin containing soap once you have applied permithrin cream as per instructions.You can repeat application after 1 week for better results.

I have taking some drugs for scabies but the itching hasn't stop yet?

Hello ,thanks for follow up.

Mainstay treatment for scabeis is permithrin cream which kills mite.

Itching will persist for few days even mite is killed by application of permithrin .This phenomenon is known as post scabitic pruritus. Anti histaminic tablets which I recommend you in our previous discussion will be helpful in such cases.

Chances of reinfection also are there if other family members are also affected.
In such cases repeat application of permithrin is recommended.

If stiil u have persistent itching sometimes doctors prescribe short course of oral steroids .However this is not required in many cases.

It will be also helpful if you let me know of medicines which you are taking.

Hi,

thanks for sending drugs.

Ivermectin is a good drug for scabeis.

Its dose for adults is 12 mg weekly for 2 weeks.

Benzyl benzoate lotion is also anti scabitic ,but it is a old molecule and not effective much .In fact it can cause irritation which can lead to more itching.

As we discussed earlier permithrin is gold standard treatment for scabeis. Probably you will get relief after permithrin application.

You can continue with hydroxhlorine tablet 25 mg tablet which is anti histaminic.

Are you saying that i should get Ivermectin for 12 mg weekly for 2 weeks. because the one i have is for just 2 weeks as well? Benzyl benzoate lotion continue or stop using it ?

It can be irritant some times leading to more itching.

Dose of Ivermectin 6 mg is the one i have with me my other observation my dick cap and scrotum has little pimple bump, how long will that go away? I have change my bed sheet already do i have to change it again ?

Hi,

Dose of Ivermectin is 12 mg every week for 2 weeks.If you have 6 mg tablet you can take 2 tablets at a time 1 week apart.

Ivermectin is preferably taken at night.

Lesion over penis and scrotum are a common manifestation of scabeis. Usually they will subside within 2 to 3 weeks ,but can persist up to 3 months.There is nothing to worry much about it.

If other members of your family are not affected ,you need not to change bedsheets.

I will still recommend you to apply permithrin cream.

Two doses of Ivermectin 12 mg is as much as effective as a single application of permithrin.

Other treatments are not effective as per my personal experience.

Ivermectin is also available in cream form in some countries.It has got good results with a single application overnight.

If you have no other options available,I will advise you to take 2 doses of Ivermectin and anti histaminics tablet and let me know after 2 weeks.
