Question: Hello doctor this for my mother query . Age 47 having hypertension and diabetes . We consulted gynaecologist for endometrial biopsy and Pap smear test. That was done 4 days ago but she said hypertension is high so better consult cardiologist for suggesting hypertension tablets because presently my mother takes olmighty h 40 it is related to cardiac . Then today we have consulted doctor took ecg and 2d echo said normal.given one extra tablet nebistar 2.5 along with olmighty h 40 but change to 20 . Olvance h 20 take for morning and nebistar 2.5 take afternoon .and given atocor10 for cholesterol but mu mother is normal cholesterol I dont know why has given said good for diabetic people. Okay my doubt is is this the right way to suggest my mother? Why 2 hypertension tablets morning and afternoon is there no medicine like one powerful tablet acts as these both tablets ?we dont want to take cholesterol tablet is this okay ? Could you explain clearly Im attaching all the reports including our gynaecology doctor assessment

Our gynecology doctor said that take stamlo beta for night time along with olmighty h 40 till consult cardiologit so till today my mom has been taking olmight h4o and stamlo beta for last 5 days.

I have gone through all the attached reports and have read your description of the issue carefully.

However, as you are more concerned about the cholesterol medication, I would address that first.

In the attached reports, there is no lipid profile. You need to upload that for me to make any useful suggestions regarding that.

There are no tests that are done for diabetes, as well.

Coming to her blood pressure, it is under good control on Olmighty H 40, as the cardiologist has recorded it to be 120/70 mmHg.

However, Olvance H 20 with Nebistar 2.5 is also a very good option, instead of Olmighty H 40 and Stamlo beta.

These medications should be adjusted upon a direct physical examination of the patient.

Get a lipid profile done and also do, FBS, PPBS and HbA1C for diabetes.

That will enable me to guide you further.

Your question about one powerful tablet is due to unjustifiable expectations and incomplete knowledge of the pharmacology. It does not work like that, we can not use all medicines however we want, there are limitations due to the therapeutic window of a drug and efficacy and safety.

I can understand your concern, it would be ideal if there is only one tablet to consume, but it is unfortunate that it is not as easy it seems.

Doctor I’m attaching in November 2017 reports and recent one op record . And one more is how to take olmighty h 20 , nebistar 2.5 and autocorrects 10 like either morning or afternoon and how long . Today biopsy results has come too and doctor said normal

As for taking olmighty H 20 and nebistar 2.5, please follow the cardiologists advice.

She can take olmighty H 20 in the morning and nebistar 2.5 in the afternoon.

She must take it lifelong, she should not stop the BP medicines ever.

It is good that the endometrial biopsy was normal.

I have reviewed all the reports you have attached here as well as the ones you sent me by mail.

Nebistar should be taken life long.

Atocor 10 need not be taken life long. Once cholesterol becomes normal, then you can stop them, but we do not know the current level of cholesterol.

It it best to get lipid profile done, after that whether she needs Atocor 10 or not can be decided.

Apart from that, the current treatment is quite good, there is no need to change the current treatment initiated by the cardiologist.

Discuss with your physician and the gynecologist and proceed further.

As of now, I don't find anything alarming or worrisome. I would strongly recommend you to go ahead with the current treatment.
